Endoscopic Ultrasound-Guided Botox Injection for Refractory Anal Fissure

Abstract: Background: Anal fissures cause severe pain and can be difficult to treat. Medical therapy is initially used, followed by sigmoidoscopy-guided botox injections if the medical therapy is not successful. With this technique, however, it is not clear whether botox is injected into the muscle layer or submucosa. Aim: To evaluate the efficacy of EUS-guided botox injection directly into the internal sphincter. Methods: Consecutive patients with chronic anal fissure refractory to conventional endoscopic botulinum tox… Show more

“…Structural gastrointestinal diseases are common globally 169,170 , with the exact prevalence and incidence likely being underreported 171 . Structural gastrointestinal diseases include diseases of the anus (external haemorrhoids, anal fissures, anorectal fistulas), rectum (internal haemorrhoids, rectal prolapse, perirectal abscess) and colon (colonic polyps, diverticular disease) 172 (Table 1).…”

“…EUS-guided botulinum toxin injection into the internal anal sphincter showed promising results in these patients with chronic anal fissures, as EUS provides direct visualization, thus ensuring injection to the internal anal sphincter only. However, with conventional endoscopic botulinum toxin injections in patients with anal fissures, injections are administered blindly without direct visualization, so they can be injected into the submucosa or muscle, thus decreasing the efficacy of treatment [9].…”
