Endoscopic ultrasound‐through‐the‐needle biopsy in pancreatic cystic lesions: A multicenter study

Abstract: EUS-TTNB with micro-forceps in PCL is feasible, safe, and has a high diagnostic yield. Future prospective studies are needed to better assess the clinical impact of EUS-TTNB on the management of PCL.

“…Pancreatic lymphoepithelial cysts are rare PCL, with no peculiar morphological characteristics that can allow the clinician to distinguish them from other more common forms, such as serous or mucinous cystic lesions. We think that improving the diagnostic yield of EUS‐FNA/B, through the use of EUS‐TTNB, as discussed, could help to avoid unnecessary surgery and its possible related complications …”

“…Recently, several studies reported the utility of EUS‐through‐the‐needle biopsy (EUS‐TTNB) for the diagnosis and risk stratification of the malignancy of pancreatic cystic lesions . This novel technique enables direct sampling from the walls of pancreatic cystic lesions using biopsy microforceps through an EUS‐FNA needle.…”

“…One multicenter study enrolling 56 patients reported that technical success was 100% and the rate of adequate sampling for histological diagnosis was 83.9%. 44 The rate of adverse events was 16.1%, but these were mild and were resolved by conservative therapy. Kovacevic et al 45 carried out EUS-TTNB in 31 consecutive patients with pancreatic cystic lesions and reported that clinical management was changed in six out of the 31 (19.4%) patients as a result of this evaluation.…”

“…In this issue of Digestive Endoscopy, Barresi et al reported a multicenter study of EUS-TTNB for PCL with a technical success rate of 100% and an adequate histological specimen rate of 83.9%. 9 Of note, the degree of dysplasia (mild or moderate) and stroma could be evaluated using EUS-TTNB specimen, which have been increasingly reported. 8 Although EUS-FNA is recommended in PCL with worrisome features, it is rarely carried out in Japan because of the risk of dissemination.…”
