Endothelial keratoplasty: DSEK/DSAEK or DMEK - the thinner the better?

Abstract: Compared with DSEK/DSAEK, DMEK may have higher clinical potential with 75% of cases reaching 20/25 or better (> or =0.8) within 1-3 months. Furthermore, preparation of isolated Descemet grafts does not require large investments and may increase overall donor tissue availability. Hence, corneal surgeons may consider 'to make the switch' from DSEK/DSAEK to DMEK.

“…The development in surgical treatment of corneal endothelial diseases, with progressive thinning of grafts and its related advantages (13,14) , leads us to conclude that transplantation with cultured en- dothelial cells may represent the next advance in this field (2)(3)(4)(5)(6)(7)(8)(9) . Animal studies have demonstrated promising results (15,16) .…”

“…1,2 Currently, descemet stripping automated endothelial keratoplasty (DSAEK) is the most popular form of endothelial keratoplasty (EK). EK has been associated with quicker visual recovery, less postoperative astigmatism, reduced risk of infection and better wound integrity giving it clear advantages over penetrating keratoplasty.…”
