Endotoxin Elimination in Patients with Septic Shock: An Observation Study

Abstract: To evaluate the effectiveness of endotoxin elimination with an adsorption column in patients with septic shock and endotoxemia. The elimination therapy was guided by a new bedside method of measuring endotoxin activity (EA). Intensive care unit (ICU) patients with septic shock and suspected Gram-negative infection were consecutively added to the study group within the first 24 h. Endotoxin elimination was performed using hemoperfusion with the Alteco LPS Adsorber. The primary endpoint was improvement in organ … Show more

“…The peptide covers the surface of a porous polyethylene matrix designed to provide an optimal binding surface. A few case series in critically ill adults have reported a decrease in endotoxin levels and a hemodynamic improvement [18][19][20]. However, the ASSET (abdominal septic shock -endotoxin adsorption treatment) multicenter RCT evaluating the feasibility of Alteco ® LPS adsorber was terminated early because of patient recruitment issues [21].…”

“…Evidence for LPS adsorbers was obtained from case reports/series [ 60 62 ] that showed a decrease in endotoxin level as well as improvement in patient hemodynamics with no significant side effects. A feasibility study (The ASSET trial) was unfortunately terminated early due to difficulty recruiting patients (NCT02335723).…”

“…Septic shock is a disease caused by infection, with abnormal blood distribution as the main feature, and with the hypoxic ischemic tissues and organs as the major pathophysiological changes ( 1 – 3 ). The gastrointestinal hypoxia remains after the blood pressure is corrected.…”
