Endovascular and Surgical Management of Intact Splenic Artery Aneurysm

Abstract: This study aims to reveal the experience with endovascular and surgical management of intact splenic artery aneurysms in our single center. MethodBetween January 2011 and June 2017, 42 patients with intact splenic artery aneurysm were enrolled in this study. Twenty patients undergoing surgical intervention were classified as the surgical group, and twenty-two patients who received endovascular repair were categorized as the endovascular group. Demographic data, preoperative comorbidities, and aneurysm anatomic… Show more

“…In recent years, endovascular therapy has been favored for being minimally invasive, offering rapid postoperative recovery, and providing a high comfort level to patients. Several endovascular treatment methods for SAA, mainly include coil embolization, placement of covered stents, plug deployment, gluing, and injection of endoluminal thrombin, polyvinyl alcohol, particles, or gel foam ( 1 , 2 , 32 , 33 ). Endovascular therapy is preferred in cases involving surgical technical difficulty and in patients with high risk of open operative.…”

“…The most commonly used technique is embolic occlusion. 17 Likewise, endovascular stent graft exclusion of SAAs, which preserve blood flow through the artery, has been reported with a higher success rate. 18 However, giant SAAs are a great challenge for endovascular treatment, because of their complex anatomy and size, which may limit the efficacy of procedures.…”

“…Open or laparoscopic splenectomy is the treatment choice for aneurysms located in the splenic hilum or immediately next to the hilum. Another option is endovascular management with stent placement or arterial embolization [6,7,14,15]. The aim of surgical treatment should be the treatment of aneurysm with the splenic preservation or the preservation of a sufficient part of the organ (not less than 25% of the volume), enough to perform the immune function.…”
