Endovascular Management of Rare Sciatic Artery Aneurysm

Abstract: The application of endovascular techniques and surgical revascularization procedures provides numerous management options for the treatment of an aneurysmal persistent sciatic artery. This case demonstrates that clinical evaluation between staged procedures may obviate the need for further intervention.

“…3,4 Cases of successful aneurysm embolization using pushable coils have been reported. [5][6][7] However, coils are hindered by difficulty with accurate placement and the possibility of unintended embolization of proximal or distal vessels. Coiled vessels or aneurysms in other locations have also been known to recanalize after a technically successful occlusion, particularly if the target is of large caliber, is undercoiled, or the coils become compacted.…”

“…Endovascular embolization is recommended for aneurysm sac exclusion as it is a safe and effective technique that decreases the chances of sciatic nerve injury, and it is less invasive than the open ligature of the aneurysm sac. 4,[6][7][8] In the complete type, it is mandatory to do a limb revascularization with open surgery bypass-''gold standard'' treatment. The most widely used procedure is the femoropopliteal bypass with vein or graft, besides the exclusion of the PSAA.…”
