Endovascular Repair with Chimney Technique for Juxtarenal Aortic Aneurysm: A Single Center Experience

Abstract: Ch-EVAR can be used to treat JRAAs with suitable anatomical conditions. However, complications of type I endoleak were not uncommon, and, therefore, further studies are required to prove its efficiency for JRAAs.

“…No strict guidelines exist. Surprisingly, the majority of CGs both visceral 6,7,21,23 and thoracic 27,32,37 were used in elective procedures. This may explain the low 30 day mortality and also implies that CGs were selected in many cases where more established endovascular techniques would have been applicable, including the use of complex custom made stent grafts.…”

“…5 Some centers use the CG technique as the treatment of choice even for elective cases. 6,7 Maybe the most important reason for this is that CGs are available off the shelf and are less prohibitively expensive than fenestrated and branched stent grafts. 7,8 The main concern about CGs has been the risk of EL-I due to the so called gutters.…”

“…Two studies did not provide aneurysm classification [22,26]. The mean diameter was reported by seven studies and ranged from 50 mm to 74 mm, and the overall mean diameter was 64 mm [11,18,19,[23][24][25]. Forty-five patients were treated in an acute setting (Table 1) [10,12].…”

“…Much of the published experience with chimneys in the setting of EVAR has consisted of small institutional case series. [8][9][10][11][12] However, the results of the Performance of the Chimney Technique for the Treatment of Complex Aortic Pathologies (PERICLES) EVAR registry continue to provide the highest quality evidence for this modality. 13 In this study, Donas et al accumulated 898 chimneys implanted in 517 patients from 2008 to 2014.…”
