Endovascular treatment for cerebral vasospasm following aneurysmal subarachnoid hemorrhage: predictors of outcome and retreatment

Abstract: We found a modest rate of functional independence at discharge in patients with aSAH who underwent endovascular vasospasm treatment. Older patients and smokers had worse functional outcomes at discharge. Initial use of angioplasty appears to decrease the need for subsequent retreatment.

“…In previous studies about ET for CV in aSAH patients, the decision-making process for treatment has mostly involved angiographic observation and clinical deterioration, indicative of DCI, not evaluation of cerebral perfusion. [ 7 , 20 ] Many studies did not consider cerebral perfusion. To improve outcomes, selection of patients based on cerebral perfusion may be needed for CV after SAH.…”

“…[ 5 ] Furthermore, changes in systemic hemodynamics may not be adequately compensated for by cerebral autoregulation impairment by focal vasospasm at the contralateral side. [ 20 ] That is why four values were obtained from the ROI and averaged as the contralateral non-vasospastic region for reduction of these biases in the present study. Verification of whether ROI averages are suitable for measuring non-spastic regions is needed in the future.…”

“…Vasospasm caused by subarachnoid hemorrhage (SAH) when aneurysms easily rupture results in a smaller inner diameter and increased blood flow velocity. 4,11,28 As a result, the WSS, calculated by formulas according to Poiseuille's law, would also in- crease. Thus, it is reasonable that high WSS in the parent artery is obtained when an aneurysm ruptures.…”

“…Patients with acute cerebral hemorrhage frequently suffer from stress ulcer complications, negatively affecting quality of life and work [11]. Intracranial hypertension is commonly observed in patients with acute cerebral hemorrhage, which damages the normal physiological functions of the brain stem and the hypothalamus, interferes with autonomic nervous system function, and results in the excessive secretion of gastric acid.…”
