“…In some cases, including our case, the fenestration appeared to occur at the connection between the superior hypophyseal artery and the Pcom. 9 , 13 , 15) In reports describing pathological analyses, a defect in the muscular layer of the vessel at each wall of the fenestration was identified. 10 , 30 , 31) It is suggested that the combination of structural wall weakness and local hemodynamic stress, especially at the proximal site, is involved in aneurismal formation.…”

“…8 In Table 2, which presents all published cases without SAH associated with an aneurysm and ICA fenestration, there is a slight approximation of numbers. Four cases were exclusively treated surgically 6,7,9,11 three interventionally 1,8,10 and one case was treated by surgical clipping and endovascular occlusion. 5 In the majority of cases with supraclinoidal fenestrations harbouring aneurysms, surgery was favoured as the first option regardless of whether the patients had SAH or not.…”

“…[2][3][4] Since 1984, 12 cases of aneurysms originating in a fenestration of the internal carotid artery (ICA) -four ruptured and eight unruptured aneurysms -have been published. 1,[5][6][7][8][9][10][11][12][13] It should be noted that in all these cases the supraclinoidal segments C6 and C7 of the ICA were concerned.…”

“…Fenestrations of the intracranial ICA appear to be rare, with only 16 cases reported previously 2)3)5-10)12-15)17)20). A review of all literature on intracranial ICA fenestration is summarized in Table 1.…”

“…Although there is no clear correlation between fenestrations and aneurysms, we tentatively suggest that fenestrations at specific locations such as the supraclinoid ICA could be associated with aneurysms. Some authors12)15)19) have also considered the possibility of an association between aneurysms and fenestrations in particular locations, such as the vertebral junction and supraclinoid ICA. The mechanism of the association between aneurysms and fenestrations at these specific locations is unknown, but we suggest that the incidence of development of an aneurysm may be increased in vertebral and supraclinoid ICA fenestrations due to relatively high hemodynamic stress because of their proximal location.…”
