Endovascular Treatment of Huge Cervicofacial Hemangioma Complicated by Kasabach-Merritt Syndrome

Abstract: A 2-month-old girl with a huge cervicofacial hemangioma complicated by Kasabach-Merritt syndrome was presented. Two weeks corticosteroid treatment was ineffective, and deterioration of coagulation parameters and compromise of the airway and left vision due to huge hemangioma prompted use of endovascular treatment. Transfemoral arterial embolization using polyvinyl alcohol particles resulted in rapid clinical improvement. Endovascular treatment should be considered as an important therapeutic option for hemangi… Show more

“…In large tumors, TAE can be a useful bridge before surgery is attempted or until the systemic treatment becomes effective [43,101,102]. There are also multiple case reports of successful treated KMS with endovascular approaches [103][104][105][106]. As mentioned in Table 1, KMS secondary to liver hemangiomas [107] and Merkel cell cancer involving the face [108] has been successfully treated with TAE.…”

“…High-dose prednisolone (2 mg/kg/day) therapy was successfully used in one case of KMS, 9) but was ineffective in many cases. 8,10,12,13) The response rate to high-dose prednisone (3-8 mg/kg/day) therapy among 11 cases of complex hemangioma was 45%, and the cure rate was 18%. 14) Interferon a2b therapy remains controversial, with a response rate of about 50%.…”

“…1) Transarterial embolization is very effective, safe, and an excellent therapeutic option. Successful embolization has been reported with several embolic materials, such as coils, 6) PVA, 8,11,12) and onyx. 15) We achieved an excellent outcome on our patient using transarterial embolization with PVA, which improved the hematological parameters dramatically.…”

“…There are also reports of using particles and liquids via the extracranial circulation to treat hemangiomas, arteriovenous fistulas, and neoplasms [5, 6]. A more recent report demonstrates the use of particles for a cervicofacial hemangioma complicated by thrombocytopenia and a consumptive coagulopathy (Kasabach-Merritt Syndrome) [7]. We describe a unique case of a combined internal and external carotid artery approach using PVA embolization, gelatin microspheres, alcohol, and temporary balloon occlusion to treat life threatening epistaxis from a skull base vascular malformation in a three-year-old girl.…”
