Enhancing Safety via Medication Self-Management and Timely Reporting

Abstract: Medication errors have been a major threat to patient safety. Current research on medication errors is largely dependent on in-hospital reports. With the rapid shift of health care to chronic condition management, there is an urgent need to investigate medication errors in the community. In this paper, we discuss that the model of medication self-management developed for outpatient settings may be used to guide the development of prevention strategies for medication errors beyond hospitals. Further, timely rep… Show more

“…Capturing individual dynamic responses to OAA treatment is critical for the development of personalized interventions [ 34 , 36 ]. Therefore, it is important to assess older adults’ OAA treatment tolerability in a timely fashion and monitor for early signs and symptoms of toxicities from home [ 37 ], to intervene early to prevent the development of severe side effects and improve patient safety and health outcomes [ 16 , 38 ].…”

“…In an observational study, the occurrence of adverse medication events in oncology patients taking OAAs was as high as 87.36% [4]. As some of the severe side effects can be di cult to manage for patients and their families at home, personalized patient education and guidance and effective patient-clinician communications are essential for improving patient outcomes and safety [5]. To achieve the goal of medication safety during TOC, patients with cancer and their families need to become "vigilant partners" in medication and toxicity self-monitoring, including timely reporting of their medication safety events or concerns to clinicians [6].…”
