Enigma of myoepithelioma at the base of tongue: A rare case report and review of literature

Abstract: Myoepithelioma, once considered to be a type of pleomorphic adenoma, is a rare benign tumor of the salivary glands. This uncommon tumor is most commonly diagnosed in the parotid gland and in the minor salivary glands of the palate. We report a case of myoepithelioma located at the base of tongue, a rare site, in a 48-year-old male, composed of spindle shaped myoepithelial cells.

They are normally located between the luminal epithelial cells and the basement membrane and have contractile function.…”

“…Myoepitheliomas are believed to arise from stem cells that differentiate into myoepithelial cells. Both benign and malignant types have been described in literature and the most common differential diagnosis is pleomorphic adenoma 5. Some authors believe that myoepitheliomas are a monomorphic, single cell variant of pleomorphic adenoma while others believe that they are a separate spectrum of benign tumours with monomorphic adenomas (composed of pure epithelial cells) at one end and myoepitheliomas (composed of only myoepithelial cells) at the other end.…”

“…Their differential diagnosis includes a wide range of both benign and malignant tumours, depending on the predominant cell type: epithelioid, spindle, hyaline, clear cell [ 8 ]. In particular, the main differential diagnosis of myoepithelioma is pleomorphic adenoma (PA) [ 3 , 9 ]. The distinction between the two is based on the fact that PA contains abundant ducts, but no more than one duct every medium to high power field or no more than one small cluster of the duct.…”

“…The distinction between the two is based on the fact that PA contains abundant ducts, but no more than one duct every medium to high power field or no more than one small cluster of the duct. Moreover, a much greater amount of stromal component is found in the PA compared to myoepitheliomas [ 9 ]. However, myoepithelioma is made up of a variety of plasmacytoid, epithelioid, and spindle-shaped cells with focally abundant mucoid stroma and may easily be confused with a PA.…”
