“…Three historical comparison studies reported the caries reductions associated with children who began rinsing as kindergartners (117)(118)(119). The results of these studies are graphed in Figure 5, in which the number of surfaces saved annually is compared to the baseline caries activity of the communities.…”

“…While these results indicate the effects to the permanent teeth of a school-based fluoride mouthrinsing program, they do not present the entire story. For the younger age group the primary teeth are also protected (90,112,117,118,120); for the older children who are no longer rinsing, a posttreatment benefit has been reported (118,121).…”

“…During the past decade there was a noticeable shift from the true randomized clinical trial employing concurrent controls to less rigorous study designs based on historical comparisons. This trend is particularlv evident in reports of fluoride mouthrinse studies in both fluoridated and nonfluoridated areas (32)(33)(34)(35)(36)(37)(38)(39).…”

“…Data from two sites of the 17-site, NIDR-supported fluoride-mouthrinsing demonstration program, conducted between 1975 and 1978, serve to illustrate this point. One program attributed a 33 percent decline in caries solelv to the use of the rinse ( Table 2 ) (37). A second +rogram, extended for a total of six years (197?1981), indicated a 55 percent savings for children in grades 1-6 ( Table 3) (38).…”

“…The review revealed serious design deficiencies in many FMR studies in both fluoride deficient (12) and fiuoridated areas (13). The most serious shortcoming was the rather common practice of basing treatment effectiveness figures on historical comparisons rather than concurrent, longitudinal control groups (14)(15)(16)(17)(18)(19)(20)(21)(22).…”
