BackgroundWe see a growing number of older adults receiving long-term care in industrialized countries. The Healthcare Utilization Model by Andersen suggests that individual need characteristics influence utilization. The purpose of this study is to analyze correlations between need characteristics and service utilization in home care arrangements.Methods1,152 respondents answered the questionnaire regarding their integration of services in their current and future care arrangements. Care recipients with high long-term care needs answered the questionnaire on their own, the family caregiver assisted the care recipient in answering the questions, or the family caregiver responded to the questionnaire on behalf of the care recipient. They were asked to rank specific needs according to their situation. We used descriptive statistics and regression analysis.ResultsRespondents are widely informed about services. Nursing services and counseling are the most used services. Short-term care and guidance and training have a high potential for future use. Day care, self-help groups, and mobile services were the most frequently rejected services in our survey. Women use more services than men and with rising age utilization increases. Long waiting times and bad health of the primary caregiver increases the chance of integrating services into the home care arrangements.ConclusionThe primary family caregiver has a high impact on service utilization. This indicates that the whole family should be approached when offering services. Professionals should react upon the specific needs of care dependents and their families.

In the face of demographic change, the informal care of dementia patients is becoming increasingly important. However, due to dementia symptoms as well as persisting care demands, this subgroup of informal caregivers is confronted with a large number of stressors resulting in chronic stress and impaired physical and mental health in many caregivers. Based on the current research on resilience (i. e., maintaining or regaining health despite stress and adversities), there is increasing interest in identifying resilience factors that may serve as resources to cope with informal care and protect caregivers against health problems. The review discusses the role of resilience factors in the association between ongoing caregiving stress and health. In analyzing the current state of research on resilience factors for dementia caregivers, we focus on the factors self-efficacy, relationship quality, and social support.

Increasing life expectancy correlating highly with polymorbidity, frequently causes recurring inpatient stays at Geriatric hospital wards. After discharge from hospital, an offer for physical and psychological after-care seems necessary for most patients. Purpose of this project was the development, implementation and primary assessment of an out-patient after-care intervention program for elderly, multimorbid patients and their relatives. During a period of eight months an interdisciplinary team offered psychological interventions as well as medical advice, nursing care and social casework. Primary goal was the investigation of health care utilization conduct and effectiveness of this after-care service in regard to sustainable stabilization of former inpatients. Preliminary results show higher demand of psychological and psychotherapeutic groups, whereas medical questions and nursing care problems were of minor concern to the patients. Contrary to our presumptions, even geriatric patients with a diagnosis of depression or anxiety disorder, showed to be reliable and constructive participants in psychological group settings. Situation and problems of (nursing) relatives could be identified accurately, although they represented an independent target group, possibly in need of more specific interventions
