We study the health trajectories of the population aged over 60, comparing between one European and two Latin American countries (Spain, Chile and Costa Rica) which have similar longevity patterns. Our focus is on functional limitation and mortality risks, considering differences by gender, education and social participation. Data come from national panel surveys (EPS, CRELES, SHARE). Multistate modelling is used to estimate transition probabilities between two health states: healthy to unhealthy, unhealthy to healthy as well as the transition to death from healthy or unhealthy states, to estimate the duration of stay in a specific state (computing healthy and unhealthy life expectancies) and the effect of the selected covariates. Results show that older Costa Ricans have the smallest gender gap in life expectancy but women have a lower healthy life expectancy compared to those in Chile and Spain. Participation in social activities leads to higher healthy life expectancy among the elderly in Costa Rica and Spain, whilst there were no relevant educational differences observed in longevity in the analysed countries. To conclude: despite the different patterns observed in health transitions and survival across the three countries, social participation is associated with greater health and longevity among people of old age, with little effect coming from educational attainment. Public policies should therefore be aimed at reducing unhealthy life years and dependency at advanced ages by promoting more engagement in social activities, especially among vulnerable groups who are more likely to experience impairment from a younger age.

Con el objetivo de perfilar el conocimiento actual disponible sobre la situación de la salud y el envejecimiento de las personas mayores en la Región de las Américas al comienzo de la Década del Envejecimiento Saludable 2021-2030, en este informe se brinda información actualizada sobre la situación de los cuidados a largo plazo en la Región. Los datos ofrecen un análisis detallado de la situación de los cuidados a largo plazo en América Latina y el Caribe, de la carga y los efectos de la dependencia de cuidados, así como de los sistemas y servicios de cuidados a largo plazo que existen en la actualidad. El acelerado envejecimiento poblacional en América Latina y el Caribe genera un aumento sustancial de la demanda de servicios de cuidados, al tiempo que se registra una disminución de la oferta de cuidados no remunerados brindados por la familia, que en la actualidad constituye el principal proveedor de atención a la dependencia. Ante tal realidad, este informe detallado permitirá fundamentar el diseño de estrategias encaminadas a formalizar esta dimensión de la atención de salud de las personas, partiendo de la idea de que establecer sistemas nacionales de atención a largo plazo resulta fundamental para proteger y promover los derechos, la dignidad y el bienestar de las personas mayores en situación de dependencia de cuidados y de sus cuidadoras y cuidadores.

This report provides updated information on long-term care in the Region of the Americas, summarizing current knowledge about the health and aging of older persons in the Region at the start of the Decade of Healthy Aging 2021-2030. The data paint a detailed picture of the long-term care situation in Latin America and the Caribbean, the burden and effects of care dependency, and current long-term care systems and services in the subregion. The rapid aging of the population in Latin America and the Caribbean is fueling a substantial increase in the demand for care services at a time when the supply of unpaid care provided by families––currently the main providers of dependency care––is shrinking. This detailed report can serve as the foundation for the design of strategies that formalize this dimension of health care, with the idea that developing national long-term care systems is essential for protecting and promoting the rights, dignity, and well-being of care-dependent older persons and their caregivers.
