Alkaline phosphatase (ALP) activity of 70 cases of non-Hodgkin's lymphomas of the B-cell type was studied. ALP activity was found in malignant lymphoma (ML), follicular, small cleaved cell (1/5 cases); ML, diffuse, small cleaved cell (3/13 cases); and mantle zone lymphoma (intermediate lymphocytic lymphoma) (2/2 cases): The ALP-positive neoplastic cells simultaneously displayed the characteristic immunophenotype of mantle zone (MZ) B lymphocytes of secondary follicle (SIg D + , BA-1 + , IL-2R + and Leu-1 +). All other B-cell lymphomas, including ML, follicular, mixed small cleaved and large cell (9 cases); ML, follicular, large cell (4 cases); ML, mixed small and large cell (7 cases); ML, diffuse, large cell (27 cases); and ML, small noncleaved cell (3 cases), were consistently negative for ALP. The present study indicates that ALP-positive lymphomas including follicular lymphomas and diffuse lymphomas may be neoplastic counterparts of ALP-positive MZ B lymphocytes.

Biopsy specimens obtained from 2 patients with monocytoid B cell lymphoma, 7 with mantle zone lymphoma, 7 with small lymphocytic lymphoma or B chronic lymphocytic leukemia, and 6 with hairy cell leukemia were investigated using an immunohistochemical method to detect their immunophenotypic characteristics. Periodate-lysine-paraformaldehyde-fixed frozen biopsies from the lymph node, peripheral blood, bone marrow, spleen, tonsil, lung, and stomach were studied. Monocytoid B cell lymphoma exhibited the immunophenotype of surface(s) IgD-/DRC-1-/Leu-1(CD5)-/Leu-M5(CD11c)-, ± on the neoplastic cells or neoplastic lesions, mantle zone lymphoma exhibited that of sIgD+/DRC-1++/Leu-1-,+/Leu-M5-, small lymphocytic lymphoma or B chronic lymphocytic leukemia that of Leu-1+/sIgD-,+/DRC-1- > +/Leu-M5-, and hairy cell leukemia that of Leu-M5++/sIgD- > > +/Leu-1- > > +/DRC-1-. We therefore suggest that these four types of lymphomas can be differentiated by a combination of anti-sIgD, DRC-1, Leu-1, and Leu-M5 monoclonal antibodies based on their immunophenotypic characteristics.

An Immunohistochemical and Ultrastructural Study of an Unusual Case of Multiple Non-X Histiocytoma

An immunohistochemical and ultrastructural study of an atypical case of multiple non-X histiocytoma was done. There was involvement of the skin, lungs, and liver in a 3-month-old male infant. Microscopic examination of the cutaneous tumors revealed a dense infiltration of cells with polymorphous large nuclei and abundant eosinophilic cytoplasm in the entire dermis. Both immunohistochemical and electron microscopic studies suggested that the tumors were non-X histiocytomas. The patient's condition deteriorated with dyspnea due to rapid enlargement of tumor masses in the liver and lungs. However, at 5 months of age, the cutaneous nodules and pulmonary and hepatic lesions showed a tendency to involute. Furthermore, at 12 months of age, they were no longer detectable. The patient at 24 months of age was well with normal development. To date, no recurrence of the disease has been observed.
