Eosinophilic granuloma of C4 causing spinal cord compression

Abstract: A rare case of vertebral eosinophilic granuloma (C4) causing spinal cord compression is reported. The clinical, histological and radiological features of this pathological entity are discussed. After surgery a complete neurological recovery was observed. The value, in selected cases, of surgical treatment with total removal of the tumour and reconstruction of the spine to ensure spinal stability and to prevent irreversible neurological deficit is emphasized.

“…In 1953, Lichtenstein grouped three diseases under the denomination "histiocytosis X ": Hand-Schuller-Christian (HSC), Letterer-Siwe (LS) diseases and eosinophilic granuloma (EG), referring to the basic subjacent proliferation of histiocytes (Langerhans'cells) in all three conditions and to the unknown ethiology 2,[13][14][15][16] . On this occasion, they were already known as variations in extents, involvement stage and localization of the same basic disease of the reticuloendothelial system, with sharp differences both in prognosis and therapy 2,6,10,14 .…”

“…The term "eosinophilic granuloma" was coined by Lichtenstein and Jaffe in 1940 when they reported two cases of solitary lytic bone lesions consisting microscopically of clusters of phagocytic cells and significative collections of eosinophils 1,13,14 . There is considerable disagreement about the nature of the histiocytoses.…”
