Migraine is a common neurological disorder considered the second most disabling condition worldwide. Its prevalence ranges from 2.6% to 21.7% in population studies. This study aimed to know the prevalence of diagnosed and undiagnosed migraine in the general Spanish adult population, their health care use, and factors related. A descriptive cross-sectional study was undertaken with 23,089 individuals >15 years from the 2017 Spanish National Health Survey. Three groups were defined: people diagnosed with migraine (DM), people reporting undiagnosed migraine (UM) and people without migraine. Sociodemographic, clinical and use of health resources data were collected. The scales Duke Social Support Index (DSSI) and General Health Questionnaire (GHQ-12) were used. Prevalence of DM and UM were determined with 95% confidence intervals. To determine the factors associated with DM and UM, a multinomial logistic regression model was used. The prevalence of DM was 8.6% (95%CI: 8.2–9), and UM, 0.9% (95%CI: 0.8–1). People with DM more frequently visited healthcare professionals (47.8%), required more supplementary tests (86.8), had a higher percentage of hospitalization (11.3%), and used emergency services (45.1%). Women had nearly three times the risk of DM and UM. Worse mental health was a risk factor for UM (OR = 1.20) and DM (OR = 1.18). The greater the work stress, the greater the risk of DM (OR = 1.12). An adequate monitoring and management of migraine in people with these characteristics could contribute to improving their quality of life and reducing costs in the system.

Introducción: con la transición demográfica y epidemiológica, se espera que la prevalencia de las patologías neurológicas aumente. En particular, Colombia no dispone de indicadores actualizados sobre la frecuencia de dichas enfermedades. Objetivo: determinar la prevalencia de 10 patologías neurológicas de relevancia (migraña, epilepsia, enfermedad de Alzheimer, ataque cerebrovascular isquémico y hemorrágico, enfermedad de Parkinson, esclerosis múltiple, miastenia gravis, meningitis, síndrome de Guillain-Barré y enfermedad de motoneurona) en pacientes con edad ≥15 años entre 2015-2017. Materiales y métodos: estudio de corte transversal a partir de fuentes administrativas, con base en los Registro Individuales de Prestación de Servicios (RIPS), consolidados por el Ministerio de Salud. Se analizaron las características sociodemográficas disponibles (sexo, grupo etario, régimen de salud, procedencia, etc.) y se ajustaron las prevalencias obtenidas según las distribuciones de la población mundial y nacional. Resultados: para el 2017, las prevalencias ajustadas a la distribución etaria mundial, por 100 000 habitantes, fueron: migraña 2170 (IC95 % 2164-2176); epilepsia 586 (IC95 % 583-589); enfermedad de Alzheimer 387 (IC95 % 384-389); ataque cerebrovascular 263 (IC95 % 260-265), específicamente, isquémico 136 (IC95 % 134-137) y hemorrágico 96 (IC95 % 95-98); enfermedad de Parkinson 91 (IC95 % 90-93), esclerosis múltiple 19 (IC95 % 18-19); miastenia gravis 11 (IC95% 11-11); meningitis 10 (IC95 % 9-10); síndrome de Guillain-Barré 9 (IC95 % 8-9); y enfermedad de motoneurona 6 (IC95 % 5-6). La mediana de variación 2015-2017 fue del 19,38%. Conclusión: Las patologías más prevalentes a nivel nacional, en orden de frecuencia, fueron migraña, epilepsia y enfermedad de Alzheimer.

Association Between Insomnia and Migraine Risk: A Case–Control and Bidirectional Mendelian Randomization Study

Background The causal relationship between insomnia and migraine is contradictory and no study has been carried out among the Chinese population to date. Methods In this case, we conducted a case–control study and a bidirectional mendelian randomization (MR) analysis to determine whether insomnia is causally related to the development of migraine. The instrumental variables for insomnia were derived from the largest genome-wide association study of 1,331,010 participants, while the genetic instruments for migraine were available from the largest meta-analysis of migraine with 59,674 cases and 316,078 controls. Results In case–control study, subjects with insomnia have significantly higher risk of migraine (OR=4.29, 95% CI: 3.21–5.74, P<0.001), compared with those without insomnia. The bidirectional two-sample MR analysis revealed that insomnia was significantly associated with higher risk of migraine (OR=1.24, 95% CI: 1.11–1.38, P=1.01×10-4), and the results were validated in the UK Biobank data. The results showed no indication for directional pleiotropy effects as assessed by the MR-Egger intercept (P>0.05). Conclusion Conclusively, our study highlighted that increased migraine risk was confined to subjects with a genetic pre-disposition to insomnia, and these findings had potential implications for improving the sleep quality to reduce the burden of migraine.
