BackgroundFacial disfigurement has been considered one of the most challenging consequences of the surgical treatment for head and neck cancer patients, mainly due to the importance of the facial region for the personal identity, body self-image and interpersonal interactions, which might affect negatively the quality of life. The aim of this study was to assess factors associated with the quality of life of subjects with facial disfigurement due to surgical treatment.Material and MethodsClinical data were retrieved from 103 patient’s medical records and quality of life data were collected using the Functional Assessment of Cancer Therapy (FACT-H&N) questionnaire. Moreover, the degree of facial disfigurement was classified by means of a specific ordinal scale.ResultsData from the FACT-H&N questionnaire showed that the domain directly related to head and neck symptoms was considered the most impacted, while emotional domain was the least affected. Lower quality of life was associated with sequels in the neck and/or lower third of the face (β=-0.39; p=0.001), a higher level of disfigurement (β=-0.29; p=0.016) and female gender (β=-0.20; p=0.038).ConclusionsDisfigurement due to surgical treatment was significantly associated with the functional dimension of the patients, especially in extensive sequels in the cervical and lower regions of the face. Tobacco use after cancer diagnosis is associated with adverse cancer outcomes, yet reliable prevalence estimates for this behavior are lacking. We conducted a systematic literature review of the prevalence of current tobacco use among individuals with a history of lung or head/neck cancer (CRD #42012002625). An extensive search of electronic databases (MEDLINE, EMBASE, Cochrane Library, CINAHL, PsycINFO, Web of Science) identified 7,777 potentially relevant papers published between 1980 and 2014, and 131 of these yielded pertinent information. Aggregating results across heterogeneous study designs and diverse patient samples, the overall mean prevalence rate of current tobacco use (mostly cigarette smoking) was 33.0% (median=31.0%). Among current tobacco users at cancer diagnosis, the mean prevalence rate of current tobacco use (mostly cigarette smoking) was 53.8% (median=50.3%). In many cases, an operational definition of “current” tobacco use was absent, and biochemical verification of self-reported smoking status was infrequent. These and other observed methodological limitations in the assessment and reporting of cancer patients’ tobacco use underscore the necessity of uniform tobacco use assessment in future clinical research and cancer care.

Introdução: O câncer bucal representa um grave problema de saúde pública no Brasil e no mundo. As informações dos casos diagnosticados por câncer, assim como os indicadores de mortalidade por essa doença, traduzem o perfil da população acometida por ela, além de auxiliar na formulação de políticas públicas para a prevenção e controle dessa enfermidade e para a melhoria da assistência oncológica. Objetivo: Descrever o perfil clínico e epidemiológico de indivíduos com câncer bucal atendidos em hospitais de referência do Estado da Bahia. Método: Estudo descritivo, retrospectivo, dos casos de câncer bucal provenientes do Sistema de Informatização de Registros Hospitalares de Câncer. Foi realizada análise descritiva das variáveis sociodemográficas, clínicas e relacionadas ao uso de tabaco e álcool. Resultados: Foram incluídos 3.309 casos, com predomínio de homens (70,16%), idade superior a 54 anos (61,43%), pardos (84,15%), com nível de escolaridade baixo (78,68%) e provenientes do interior do Estado (73,20%). O estadiamento IV foi o mais frequente (55,23%), o tipo histológico foi carcinoma de células escamosas (83,61%) e a localização primária do tumor na língua (31,10%). Ao final do primeiro tratamento, apenas 9,31% encontravam-se sem evidência de doença/em remissão completa, enquanto 32,47% foram a óbito. A maioria dos indivíduos apresentava história de consumo de álcool e uso de tabaco. Conclusão: Na Bahia, o câncer bucal tem sido diagnosticado tardiamente e tem acometido mais homens, acima dos 54 anos, de cor parda, com baixa escolaridade, resultando em um percentual importante de óbitos.
