Objectives: to reflect on the implementation of an integrated strategy to eradicate the Acquired Immunodeficiency Syndrome (AIDS) epidemic in Guinea-Bissau by 2030. Methods: a reflective study. Reflection: Guinea-Bissau is a Portuguese-speaking country located in Sub-Saharan Africa, in constant political and economic instability. Among its characteristics are sociocultural diversity and high rates of morbidity and mortality from causes related to infection by the Human Immunodeficiency Virus. In the quest to eradicate the AIDS epidemic by 2030, instituted especially by the United Nations, it is noted that political and socio-cultural factors transformed eradication of the AIDS epidemic by 2030 into a utopia. Final Considerations: international strategies, although ambitious, are considered opportunities for countries to propose and build public policies capable of changing the existing reality.

Introducción: Un mayor conocimiento y difusión acerca del comportamiento epidemiológico de la infección por VIH en el Departamento de Itapúa genera una evidencia significativa para la toma de decisiones a nivel regional. Objetivo: Caracterizar epidemiológicamente a pacientes con VIH que forman parte del PRONASIDA del Hospital Regional de Encarnación desde el año 2017 a 2021. Material y Métodos: Estudio transversal descriptivo en pacientes con VIH captados por el PRONASIDA del Hospital Regional de Encarnación, 7ª Región Sanitaria, durante los años 2017-2021 (n=375). Los datos se expresaron en medidas de tendencia central y dispersión, y tabla de proporciones. Resultados: El mayor número de casos (80) se evidenció en los años 2018 y 2019. El rango etario de 20-29 años reportó el mayor número de casos, 20% (75), con predominancia del sexo femenino 64% (229), el 47% (177) procedente de Encarnación. Se halló un 43% (106) de casos de sida con CD4 menor a 200 células/mL, la mediana de la carga viral fue de 96.030cél/mL con un rango intercuartílico de 401.884,5cél/mL. Conclusión: Se identificó la disminución de nuevos diagnósticos en el año 2020, lo cual guarda relación con las medidas sanitarias a causa de la pandemia por COVID-19. Las características analizadas mostraron predominancia del sexo femenino en grupos etarios en edad joven con procedencia de Encarnación. Se considera importante fortalecer el primer nivel de atención en Itapúa a fin de evidenciar las brechas con el objetivo de ampliar el acceso para captación de pacientes y asegurar un diagnóstico precoz.

HIV prevalence and associated risk factors among men who have sex with men in three regions of Paraguay, 2020

Background Our study aimed to measure HIV prevalence and associated risk factors among men who have sex with men (MSM) in three regions of Paraguay in 2020. Methods MSM were recruited for cross-sectional surveys in three regions of Paraguay using respondent-driven sampling. Interview were conducted face-to-face to collect demographic characteristics and risk and preventive behaviors. The analysis assessed HIV prevalence and associated risk factors in the three samples of MSM within each region. Results A total of 1,207 MSM were recruited, including 559 in Asunción-Central, 245 in Alto Paraná, and 403 in Caaguazú. HIV prevalence was 24.2% (95% CI 20.6–27.9) in Asunción-Central, 10.2% (95% CI 6.7–14.6) in Alto Paraná, and 3.2% (95% CI 1.7–5.4) in Caaguazú. In Asunción-Central, associations with HIV were age ≥25 years (1.86, 95% CI 1.15–3.00), being employed (1.82, 95% CI 1.07–3.11), self-reporting as homosexual (1.90, 95% CI 1.06–3.43), having sex with a known HIV-positive partner acquisition (4.19, 95% CI 2.37–7.43), self-perceived as being at higher risk for HIV acquisition (4.15, 95% CI 2.54–6.77), and able to access condoms and lubricants (1.82, 95% CI 1.08–3.05). In Alto Paraná, associations with HIV were self-reporting as homosexual (4.33, 95% CI 1.19–15.65) and having higher HIV knowledge (2.53, 95% CI 0.97–6.61). In Caaguazú, associations with HIV were self-reporting as homosexual (7.06, 95% CI 1.53–32.46) and being diagnosed with depression (4.68, 95% CI 0.89–24.43). Conclusions HIV prevalence among MSM in Paraguay varied by region, being highest in the capital and major metropolitan area of Asunción-Central, followed by the border area of Alto Paraná. While being self-identified as homosexual was associated with HIV in all three regions, other associations differed, indicating prevention programs need to be tailored to the locale.
