ObjectivesNumerous studies have reported the epidemiological and clinical features of Malta fever incidence in Iran. Review and synthesis of the related literature through meta-analysis can provide an appropriate measurement for aforementioned indices. Therefore, the present study aimed to determine the epidemiological and clinical features of people with Malta fever in Iran.MethodsThe required documents were obtained through searching national and international databases. In each study, standard deviation of the indices was calculated using binomial distribution formulas. Finally, the heterogeneity index was determined between studies using Cochran (Q) and I2 tests.ResultsCombining the results of 47 articles in the meta-analysis indicated that 57.6% (55.02–60.1%) and 42.3% (49.8–44.9%) of the patients were male and female, respectively. Most of the patients lived in rural areas; 68.4% (63.6–73.2%) compared to 31.4% (26.7–36.3%). In addition, 20.8% (17.4–24.2%) of the patients were ranchers and farmers, 16.9% (14.5–19.4%) were students, and 31.6% (27–36.2%) were housewives. Of the patients studies, 50.5% (35.6–65.2%) experienced contact with animals and 57.1% (46.4–67.9%) used unpasteurized dairy products. Fever, joint pain, and sweating were detected among 65.7% (53.7–77.8%) and 55.3% (44.4–66.2%), respectively.ConclusionThe present study revealed that the frequency of male patients with brucellosis was considerably more than that of female patients. The number of patients with Malta fever in rural areas was significantly more than in urban areas. High-risk behavior, unprotected contact with animals, and using unpasteurized dairy products were among the most significant factors affecting Malta fever incidence in Iran. Fever, joint pain, and sweating were detected among most of the patients with Malta fever.

Background and Purpose: Investigation of the seasonal pattern of brucellosis occurrence may help us to determine the etiology of disease. Seasonal pattern of brucellosis in different parts of Iran has been reported by several studies. Combining the results of these studies provide a reliable estimation of a total seasonal pattern of this zoonosis disease. This study aims to determine the overall pattern of brucellosis in Iran using meta-analysis method. Materials and Methods:Relevant evidence was identified searching the national and international databanks. Eligible articles were entered into the final meta-analysis after comprehensive review of evidence as well as quality assessment. We also calculated the standard error of the incidence according to binomial distribution formula. Because of the significant heterogeneity observed between the results, random effects model was used to combine the results. All data analyses were conducted using Stata software. Results:In total, 26 studies were entered into this systematic review including 17,311 subjects.Incidences (95% confidence intervals) of brucellosis in Iran during spring, summer, autumn and winter were estimated as of 34.4% (30.3-38.6), 33.2% (30.7-35.7), 16.4% (13.3-19.5) and 14.9% (12.7-17.1), respectively. Conclusion:This meta-analysis showed that the highest incidences of brucellosis are occurred during spring and summer, while the lowest incidences are occurred during winter and autumn.

Brucellosis leads to some systemic symptoms in humans but breasts are rarely affected, while it has frequently in animals. Brucellosis is endemic in Iran. A 30-year-old lady, living in rural area in West Azerbaijan, with fever, trembling and inflammation on the one side of left breast referred to infectious clinic of Taleghani hospital. The patient declared that had fever, weakness, arthritis and loss of appetite 1 week prior to the visit. Ultrasound was requested and showed mastitis. Antibiotics in beta-lactam groups had no effect on the patient. Serology tests, Wright, Coombs-Wright and 2-mercaptoethanol (2ME) was positive with the titer of 1/640, 1/1280 and 1/640, respectively. Treatment method was a combination of doxycycline, gentamicin and rifampin. The patient was under observation for 2 months until her ultrasound results changed to normal. It is concluded that the probability of Brucella mastitis should be considered as a differential diagnosis in endemic areas. Other differential diagnoses can be inflammatory carcinoma and simple mastitis.
