Acute kidney injury (AKI) is a complex condition that can occur in both community and hospital settings and has many aetiologies. These aetiologies may be infectious, toxic, surgical, or related to the different management methods. Although it is a major public health problem worldwide, it must be emphasised that both its incidence and mortality rate appear to be very high in sub-Saharan African (SSA) countries compared to developed countries. The profile of AKI is very different from that of more developed countries. There are no reliable statistics on the incidence of AKI in SSA. Infections (malaria, HIV, diarrhoeal, and other diseases), nephrotoxins, and obstetric and surgical complications are the main aetiologies in Africa. The management of AKI is costly and associated with high rates of prolonged hospitalisation and in-hospital mortality.

Numerous studies have shown a correlation between dengue virus (DENV) infection and kidney disease. However, there is no existing meta-analysis on the prevalence of kidney diseases in the dengue population. A thorough systematic review and meta-analysis were undertaken to determine the prevalence of renal problems in people with DENV infection in order to fill this knowledge gap. A rigorous electronic literature search was carried out up to 25 January 2023 in a number of databases, including ProQuest, EBSCOhost, Scopus, PubMed and Web of Science. The search aimed to find articles that reported on the prevalence of kidney diseases in patients with DENV infection. Using the modified Newcastle–Ottawa Scale, the quality of the included studies was assessed. The meta-analysis included a total of 37 studies with 21 764 participants reporting on the prevalence of acute kidney injury (AKI) in individuals with DENV infection. The pooled prevalence of AKI in dengue patients was found to be 8% (95% confidence interval 6 to 11), with high heterogeneity across studies. The studies included are of moderate quality. The study revealed a high AKI prevalence in dengue patients, underlining the need for regular renal examination to detect AKI early and reduce hospitalization risk. Further research is needed to understand the dengue–kidney relationship and develop effective management strategies.

Effects of continuous renal replacement therapy on Apache-II score, creatinine, and urea nitrogen levels in patients with acute kidney injury

Objective: To analyze the effects of continuous renal replacement therapy (CRRT) on acute physiology and chronic health scoring system II (APACHE-II) score, creatinine, and urea nitrogen levels in patients with acute kidney injury (AKI). Methods: Medical records of 79 patients with AKI treated in Shandong Provincial Third Hospital from January 2019 to January 2021 were retrospectively divided into two groups based on the received treatment. Of them 37 patients received intermittent hemodialysis (IHD) treatment (control group) and 42 patients received CRRT (observation group). Clinical efficacy, survival rate, severity of disease, renal function and serum electrolytels and fluid balance were analyzed. Results: After the treatment, the total efficacy of the observation group was 95.24%, and the 6-month survival rate was 97.62%, which was higher than 81.08% and 83.78% in the control group, respectively (P<0.05). The Apache-II score of the observation group was (15.76±4.29), which was lower than that of the control group (23.62±5.37). Levels of creatinine, urea nitrogen, and serum levels of potassium (K+), chlorine (Cl-) and sodium (Na+) in the observation group were lower than those in the control group (P<0.05). Conclusion: CRRT can achieve significant results in the treatment of patients with AKI, help to improve the curative effect, survival rate, alleviate the severity of the disease, recovery of renal functions, the recovery of serum electrolytels and fluid balance. doi: https://doi.org/10.12669/pjms.39.1.6591 How to cite this: Hou H, Li L.
