Epidemiology of IgA nephropathy in central and eastern Kentucky for the period 1975 through 1994. Central Kentucky Region of the Southeastern United States IgA Nephropathy DATABANK Project.

Abstract: Population-based incidence data for IgA nephropathy (IgAN) are available for some countries but not for the United States. The purpose of this study was to determine the incidence of IgAN in central and eastern Kentucky for 5- and 10-yr periods between 1975 and 1994 and to examine differences among patient groups between those periods. The incidence of IgAN was 5.4 cases per one million population per year (MPPY) for period 1A (1975 through 1979), increasing to 12.4 cases per MPPY for period 2B (1990 through 1… Show more

“…Some of this variability can be explained by differences in health screening policies and biopsy practices between these regions, but genetic factors also likely contribute [15]. The incidence in the United States has been estimated at 1 per 100,000 person-years [16]. Distribution between the sexes varies by region for reasons not yet defined; the male:female ratio is 2-3:1 in North America [16][17][18] and Europe [19], but about 1:1 in East Asia [20].…”

“…Crescentic disease, often accompanied by accelerated and irreversible loss of renal function if not treated aggressively, may be the initial manifestation of IgAN or may occur superimposed on known, previously stable, mild disease. (44) Alternately, acute kidney injury can present with minimal glomerular injury when substantial hematuria leads to damage of tubular epithelial cells or obstruction of the tubules by red-cell casts. This circumstance is a reversible phenomenon, and renal recovery usually occurs with supportive measures.…”

“…Overdose burden is elevated in the United States' rural communities (1), including Appalachian Kentucky (2,3). In 2021, Kentucky had the fth-highest ageadjusted fatal overdose rate in the U.S., at 1.7 times the national average (4).…”
