Epidemiology of pneumococcal serotype 6A and 6C among invasive and carriage isolates from Alaska, 1986–2009

Abstract: We investigated serotype 6A/6C invasive pneumococcal disease (IPD) incidence, genetic diversity, and carriage before and after 7-valent pneumococcal conjugate vaccine (PCV7) introduction in Alaska. IPD cases were identified through population-based laboratory surveillance. Isolates were initially serotyped by conventional methods, and 6C isolates were differentiated from 6A by polymerase chain reaction. Among invasive and carriage isolates initially typed as 6A, 35% and 50% were identified as 6C, respectively… Show more

“…Similar results have been reported from overseas and local studies [12][13][14][15][16][17]23]. Serotype 6C has been reported to be disease causing with an increase in prevalence of IPD caused by 6C in the post PCV era [32][33][34][35]. Serotypes 6A and 6C are structurally similar with in-vitro immunological crossreactivity, the inclusion of 6A in the PCV13 has been reported to induce cross-functional anti-6C opsonophagocytic responses [36][37][38].…”

“…We have shown in this study that PCV-7 induces high total and functional antibodies against 6A [10] , which also persist at high levels up to the pre-school years ( Figures 3 and 4 ). There is some evidence from both the US and the UK that cross-reactive antibodies against serotype 6A are effective in preventing carriage and disease [7] , [16] , [22] , [25] , [26] , although serotype 6C isolates were initially misclassified as 6A, and changes in IPD rates caused by 6A may also depend on other factors than vaccination [27] . A completely different picture can be seen for serotype 19A.…”

“…Serotype 6C was the most frequently carried serotype in our study ( Table 2 ). In Alaska and the UK, carriage of serotype 6A declined following 7vPCV programs, while the proportion of 6C carriage isolates increased [ 7 , 19 ]. While data on carriage of 6A in WA are not available for the pre-7vPCV era, it is possible that a similar “replacement” took place following the 7vPCV program, given that 6C was the most commonly carried serotype in our study.…”
