In dogs with symptomatic or asymptomatic leishmaniosis, Leishmania infantum appears to induce a mixed Th1/Th2 immune response that in the sick dog may eventually result in tissue damage via different pathomechanisms, notably granulomatous inflammation (eg, nodular dermatitis, osteomyelitis), immune complex deposition (eg, glomerulonephritis), and/or autoantibody production (eg, polymyositis). This is a compensatory but detrimental mechanism generated mainly because of the insufficient killing capacity of macrophages against the parasite in the susceptible dog. Clinical disease is typically exemplified as exfoliative and/or ulcerative dermatitis, with or without nasodigital hyperkeratosis and onychogryphosis, glomerulonephritis, atrophic myositis of masticatory muscles, anterior uveitis, keratoconjunctivitis sicca, epistaxis, and/or polyarthritis, appearing alone or in various combinations. The pathogenesis of these clinical conditions has recently been highlighted, to a greater or lesser extent. The usually subclinical conditions expressed as chronic colitis, chronic hepatitis, vasculitis, myocarditis, osteomyelitis, orchiepididymitis, and meningoencephalomyelitis, though uncommon, are of pathologic importance from a differential point of view. The leading cause of death among canine leishmaniosis patients is chronic proteinuric nephritis that may progress to end-stage kidney disease, nephrotic syndrome, and/or systemic hypertension. However, even the asymptomatic proteinuria, when profuse, may be a serious problem because it predisposes to arterial thromboembolism and eventually contributes to the deterioration of the body condition.

Visceral leishmaniasis (VL) is a serious public health problem in Brazil and worldwide. Despite the euthanasia of dogs serologically positive, such action has not solved the endemic in Brazil. A risk area for VL involves the presence of the vector, the occurrence of canines and the record of human cases. The factors that have favored the persistence and spread of VL in Brazil are related to the predatory action of man on the environment, to the migratory movements and rural exodus, and a close coexistence of man and animals. Thus considering the epidemiological chain of VL, one fact seems clear: we do not know yet the true extent of the participation of the dog in the infectious cycle of VL. The clinical disease is an important indicator of the extent of the problem in an endemic area. The immunological events are complex and involve resistance and susceptibility to canine visceral leishmaniasis (CVL). One aspect in CVL is that many symptomatic dogs underwent xenodiagnosis not infect the vector. In such cases the pattern recognition receptors CD11b+, TLR2+, and NO present higher values for dogs with results in immunohistochemistry of skin and xenodiagnosis negative (IMH − /XENO − ) than dogs with immunohistochemistry in skin and xenodiagnosis positive (IMH + /XENO + ), suggesting that innate immunity modulates the competence of the dog to infect the vector. The organic response in CVL varies from individual to individual and within the same individual, with a strong evidence of being organ-specific. Only 20% of asymptomatic dogs have parasites in the skin and 15% are able to recover from clinical signs and eliminate the parasites spontaneously. In this review, we analyze the epidemiology of visceral leishmaniasis and clinical, immunological and pathological conditions that can contribute to understanding the role of dogs in transmission of visceral leishmaniasis.

ResumoA leishmaniose visceral canina é uma doença infecciosa crônica considerada endêmica em algumas regiões do estado de São Paulo e, apesar de ser uma enfermidade sistêmica, no cão, a maioria dos sinais clínicos são dermatológicos. Foram avaliados trinta cães com diagnóstico de leishmaniose, da região de Araçatuba, pelos exames parasitológico (punção aspirativa por agulha fina de linfonodos) e sorologia (IFI). Os cães apresentando unicamente sinais dermatológicos formam divididos em dois grupos: um grupo com descamação e outro com ulceração. Houve diferença estatística entre a população de células CD3+ e CD79a+ dentro do mesmo padrão dermatológico, o mesmo ocorreu com a população de CD79a+ entre os padrões descamativo e ulcerativo (p<0,05). Thirty dogs with positive results in serology (IFI) and parasitological exams (fine needle aspiration) for leishmaniasis from Araçatuba city were evaluated. They only showed dermatological signs and were divided in two groups, one with exfoliative (n=15) and other with ulcerative (n=15) lesions. There was significant statistical difference between CD3 and CD79a population in the same dermatological pattern, and with CD79a population between exfoliative and ulcerative patterns (p<0,05).
