Epidermoid cysts in kidney transplant patients receiving tacrolimus

Abstract: We present a series of three kidney transplant patients developing epidermoid cysts after receiving oral tacrolimus for long‐term prevention of rejection of the allograft. Cyclosporin A has been known to show this cutaneous adverse drug reaction, but this is the first series of patients with epidermoid cysts following tacrolimus to the best of our knowledge.

“… 1 More recently, the publication of Verma et al highlights the effect of prolonged use of tacrolimus, an immunosuppressant used in the prevention of transplant rejection, in the occurrence of renal epidermoid cysts. 5 Our patient is relatively younger than the average age found in the literature and has no notable pathological history. There is no evidence of lithiasis or renal transplantation.…”

“…Eruptive infundibular cysts have been reported to occur following cyclosporine and tacrolimus. 10 Eruptive infundibular cysts are epidermal inclusion cysts that differ from EVHC by the absence of sebaceous glands and vellus hairs. Whether the occurrence of EVHC in our patient was related to long term use of methotrexate or is related to the underlying morphea is a matter of speculation.…”

“…In an earlier report, 9 EVHC were reported in a patient with a past history of psoriatic arthritis who was treated with infliximab and topical clobetasol for 6 years but the relationship of drugs or underlying autoimmune disease to EVHC was not addressed. Eruptive infundibular cysts have been reported to occur following cyclosporine and tacrolimus 10 . Eruptive infundibular cysts are epidermal inclusion cysts that differ from EVHC by the absence of sebaceous glands and vellus hairs.…”
