Epithelial tumors metastatic to the uterine cervix. A study of 33 cases and review of the literature

Abstract: ETASTASIS of malignant epithelial tumors to the M uterine cervix is a rare occurrence, and the earliest report was in 1880 by Torock and Wittelshofer.' Although the literature contains references to small numbers of cases, many of these are poorly documented and distinction is blurred between direct spread of an adjacent primary tumor and true metastasis from a distant Standard textbooks of gynecologic pathology4 acknowledge the rarity of the condition, and devote little space to its consideration. “…The reasons for the relative rarity of metastases to the uterine cervix have not been fully elucidated, but the high fibromuscular tissue content and low degree of vascularity of the cervix are considered to be an unfavorable medium for metastatic growth (33). In addition, the cervix has lymphatic channels that drain centrifugally, and metastasis via this route occurs only when distant channels are blocked by the tumor and do not allow for retrograde flow (34).…”

“…Other known sites of primary tumours that can cause metastases to the uterine cervix are urethra, melanoma, pancreas, lungs and breast [13]. After investigation of 33 cases of distant metastasis to the cervix, Limoine and Hall found four or 12.1% of all to be from primary breast cancer [14]. Only 35 breast cancer cases with cervical metastasis were reported in the literature.…”

“…The infrequent occurrence of cervical metastasis from EOC has already been previously reported in several small series. [13][14][15][16][17][18] The coexistence of ovarian and endometrial carcinoma, considered to be present in about 3-10% of EOC patients, 19,20 can be considered as another reason for hysterectomy. These findings are in line with our results showing coexistence in 8.7% of the patients.…”
