Epstein-Barr virus-associated lymphoproliferative lesions presenting as a hydroa vacciniforme-like eruption: an analysis of six cases

Abstract: Three different clinical courses were found in six patients with an HV-like eruption associated with chronic EBV infection: (i) spontaneous remission; (ii) clearing after photoprotection; and (iii) continuous recurrence irrespective of sun exposure. It is possible that there are two patterns of HV-like eruption associated with chronic EBV infection. One is characterized by recurrent necrotic papulovesicles of the face and the other by nodules and facial swelling. It was demonstrated that the skin lesions could… Show more

“…In some cases, systemic symptoms, including fever, wasting, lymphadenopathy, and hepatosplenomegaly, may be present. (39)(40)(41)(42) In HV-like eruptions, both T and NK cells infiltrate the superficial dermis and the subcutaneous tissue. (10) Recently, we reported three cases of HV-like lymphoma with EBV-infected cd T cells using the FISH assay.…”

“…10 In some of these patients, systemic symptoms including fever, wasting, lymphadenopathy, and hepatosplenomegaly have been recorded. [24][25][26] Severe mosquito bite allergy was determined to be associated with EBV ϩ NK cells, but rarely with EBV ϩ T cells, and to progress into overt lymphoma or leukemia in the long-standing clinical course. 9,27 These EBV ϩ cutaneous diseases had the same geographic distribution as the other EBV ϩ T/NK-cell lymphomas and LPDs among east Asians and Native Americans in Central and South America and Mexico, 8 and were encountered as a part of the initial and accompanying symptoms of the systemic EBV ϩ T/NK-LPDs.…”

“…It has been proposed that the most useful criterion to separate these 2 entities is monoclonality of the TCR genes. 4,21,22 Nevertheless, the differential diagnosis seems to be arbitrary, posing serious diagnostic and therapeutic problems for pathologists, dermatologists, and hematologists.…”
