“…Az adenovírus A31, B3, B7, C5 és D8-as szerotípusaival szemben a CMX001 5-200-szoros aktivitást mutat [62]. Egy súlyosan immunszuppreszszált, HSCT-n átesett gyermek disszeminált adenovírus-fertőzését sikeresen eradikálták CMX001-gyel, miután az intravénásan adott cidofovir hatástalannak bizonyult [63]. Az emberi adenovírusok modellként szolgálnak genetikai kutatásokban és génterápiában. Az általuk immunkompetens egyénekben okozott légúti, gyomor-bél rendszeri és szemfertőzések többnyire szövődménymentes lefolyá-súak. Lappangó állapotukból azonban reaktiválódhatnak és akár halálos opportunista fertőzéseket okozhatnak immunszuppresszált egyénekben. Az immunszuppresszió okától függően különféle szerotípusú adenovírusok egyedül vagy kombinációkban eltérő szerveket betegíthetnek meg. Súlyos kombinált immundefi cientiában szenvedő gyermekekben tüdő-és májgyulladást, vérzéses hólyaghurutot okozhatnak az A-és C-species szerotípusai. A szerzett immunhiányos állapotok közül gyermekkori haemapoeticus őssejt átültetését követő légúti, bélrendszeri és húgyúti gyulladásokban a 31-es típus gyakori. Szervátültetést követően a B-species szerotípusai okozhatják az átültetett szerv pusztulását. AIDS során az F-speciesbe sorolt új szerotípusok, valamint a D-species tagjai okoznak bélgyul-ladást, a B-species rekombináns szerotípusai pedig húgyúti fertőzéseket. Lymphomák, daganatok, szisztémás lupus erythematosus rosszabbodását adenovírusok okozta immunszuppresszió elősegítheti. Az opportunista adenovíru-sok diagnosztikájában a szerológiai eljárások nem megbízhatóak, a vírus-DNS, illetve a kópiaszám nyomon köve-tése célszerű. Kezelésre cidofovirszármazékok, ribavirin, ganciclovir, vidarabin, mikro-RNS alkalmas. Orv. Upper respiratory, gastrointestinal or ocular infections usually have mild course without any major complication in immuncompetent individuals. However, reactivation from latency in immuncompromised patients may lead to death. Depending on the underlying diseases, different adenovirus serotypes damage different organs. In children with severe combined immunodeficiency syndrome, serotypes of species A and C induce lung, liver or bladder infl ammation. Paediatric hematopoietic stem cell transplantation is frequently followed by serotype 31-induced pneumonia, enteritis, cystitis. B serotypes can destroy transplanted organs. In AIDS patients, D and novel F serotypes cause enteritis. Recombinants of B serotypes induce urinary tract infections. Progression of lymphomas, tumours, and systemic lupus erythematosus might be facilitated by immunsuppressive effects of adenoviruses. As far as the diagnostic work-up of adenoviruses, detection of viral DNA and virus copy number is predictive, while serology testing is quite unrealiable. For treatment, cidofovir derivates, ribavirin, ganciclovir, vidarabine and microRNA have been used. Orv.
