Introdução: Escoliose Idiopática do Adolescente (EIA) é uma doença de causa desconhecida e diagnosticada em indivíduos com idade igual ou superior a 10 anos. A base do tratamento é uma avaliação fidedigna, compreendendo anamnese, exame físico e obtenção de radiografia. A avaliação clínica compreende o teste de Adams, análise postural e cálculo do ângulo de Cobb. Objetivo: Analisar alterações clínicas e radiográficas pré e pós-tratamento conservador em paciente portador de EIA. Métodos: Os recursos terapêuticos utilizados foram técnicas de iso-streching, terapia manual e órtese toracolombossacra. Resultados: Ao finalizar 30 sessões, os dados foram analisados por comparação entre as avaliações pré e pós-tratamento fisioterapêutico, e teve como parâmetros os resultados do teste de Adams, do ângulo de Cobb e da análise fotográfica. A paciente apresentou diminuição da gibosidade no teste de Adams, redução do ângulo de Cobb em 6º e melhora postural. Conclusão: Com o tratamento conservador da EIA, obtiveram-se resultados clínicos e radiográficos significativos.

Objective: To create a new instrumentation principle for the treatment of adolescent idiopathic scoliosis (AIS) and similar conditions. Methods: A new fixation format was created for the treatment of AIS using 3rd generation instrumentation in short, apical and single or multiple fixations. In patients presenting one scoliotic curve, one fixation is performed. In patients with two curves, two fixations are performed, and in those with three curves, three fixations are performed. To evaluate the new method, a retrospective study of 54 patients who had already undergone surgery for AIS and similar conditions using this innovation was conducted. Results: In this series, average corrections of 72% in the proximal curve, of 83% in the thoracic curve, and of 85% in the thoracolumbar curve were verified. Conclusions: It was concluded that the new fixation principle in the treatment of AIS and similar conditions using short, apical and multiple fixations presented excellent correction, better still in the curves that were more distal in relation to the spine. There was a significant statistical difference between the percentages of curvature correction. Level of Evidence III; Retrospective study.

Idiopathic scoliosis: evaluation of loss of correction in postoperative follow-up

Objective: To evaluate the postoperative loss of scoliosis correction using third-generation instrumental, comparing the immediately postoperative period, and the last visit of the patients operated on from 2002 to 2010. Method: This was a cross-sectional study, conducted by analysis of medical records, in which 45 patients undergoing scoliosis correction were included. Variables were evaluated preoperatively, immediately postoperatively and in the last follow-up visit after surgery. Statistical analysis of data was performed in PASW program, with a significance level of 95%. Results: Among the 45 patients studied, 88.9% were female and 82.8% were in the skeletally immature group. The mean pre-operative Cobb was 57°, the postoperative was 6.5° and at the last visit, it was 7.04°. There was no statistic difference between postoperative Cobb angle and that at the last examination (p = 0.176). Conclusion: There was no significant loss of scoliosis correction loss between the immediate postoperative and the final radiographic evaluation.Keywords: Scoliosis/diagnosis; Scoliosis/surgery; Scoliosis/radiography; Surgical procedures, operative; Bone screws.
