SUMMARY: The determination of variables such as age, height, ethnicity and sex are extremely important to the identification of biological findings, especially in accidents with fatal victims. The diagnosis of sex can be 100 % certain for cases in which the skeleton is complete and in a good state of conservation, the individual is an adult and the morphometric variables of the population to which the individual belongs are known.: The aim of the present study was to perform a morphometric evaluation of the proximal femur and compare measurements between males and females. Sixty pairs of femurs were acquired from the Department of Animal Morphology and Physiology of the Universidade Federal Rural de Pernambuco. The bones were measured with the aid of the ImageJ software program and the following variables were compared between bones from male and female cadavers: DFH -diameter of femoral head on the craniocaudal and sagittal axes; DFN -diameter of femoral neck on the craniocaudal and sagittal axes; FNLfemoral neck length; ILL -intertrochanteric line length. The morphometric variables had the following mean values in the male bones: DFH-craniocaudal axis -45.10 ± 0.35 mm; DFH-sagittal axis -48.27 ± 0.35 mm; DFN-craniocaudal axis -33.21 ± 0.40 mm; DFN-sagittal axis -29.96 ± 0.05 mm; FNL -31.71 ± 0.05 mm; ILL -66.47 ± 0.59 mm. The mean values for the female bones were as follows: DFH-craniocaudal axis -40.68 ± 0.20 mm; DFH-sagittal axis -42.61 ± 0.20 mm; DFN-craniocaudal axis -29.11 ± 0.03 mm; DFN-sagittal axis -26.05 ± 0.04 mm; FNL -31.10 ± 0.04 mm; ILL -60.80 ± 0.41 mm. With the exception of the femur neck length, all variables measurements were significantly larger (p < 0.0001) on the male bones. The present findings demonstrate that the femur bone, particularly the proximal portion, exhibits important sexual dimorphism and has high potential for forensic purposes. Identifying native femoral version from proximal femoral landmarks would be of benefit both for preoperative assessment as well as intraoperatively. To identify potential markers for femoral anteversion, an empirical framework was developed for orientation-independent analysis of the proximal femur from pelvic CT to allow for segmentation of the proximal femur into five constituent regions: Femoral head, femoral neck, greater trochanter, lesser trochanter and femoral shaft. The framework is based on the identification of differences in the radius of curvature at anatomic zones of transition between regions of the proximal femur, followed by non-linear geometric shape fitting. The framework is applied to 86 proximal femurs segmented from pelvic CTs, with at least 2 cm of proximal femur remaining below the lesser trochanter, obtained for non-musculoskeletal pathology to investigate potential proximal femoral markers for native femoral version. The analysis of the proximal femur suggests a fixed relationship between the maximal femoral canal diameter 1 cm proximal to the base of the lesser trochanter from the center of the greater trochanter and the femoral neck axis of 4.13° +/- 4.99°. Further full-length femoral studies are needed to confirm the relationship of the maximal canal diameter as a proxy for native femoral anteversion. Published by Wiley Periodicals, Inc. The assessment of sex is crucial to the establishment of a biological profile of an unidentified skeletal individual. The best methods currently available for the sexual diagnosis of human skeletal remains generally rely on the presence of well-preserved pelvic bones, which is not always the case. Postcranial elements, including the femur, have been used to accurately estimate sex in skeletal remains from forensic and bioarcheological settings. In this study, we present an approach to estimate sex using two measurements (femoral neck width [FNW] and femoral neck axis length [FNAL]) of the proximal femur. FNW and FNAL were obtained in a training sample (114 females and 138 males) from the Luís Lopes Collection (National History Museum of Lisbon). Logistic regression was used to develop a model to predict sex in unknown individuals. The logistic regression model correctly predicted sex in 85.3% to 85.7% of the cases. The model was also evaluated in a test sample (96 females and 96 males) from the Coimbra Identified Skeletal Collection (University of Coimbra), resulting in a sex allocation accuracy of 80.1% to 86.2%. This study supports the relative value of the proximal femur to estimate sex in skeletal remains, especially when other exceedingly dimorphic skeletal elements are not accessible for analysis.
