Objective. Management of systemic lupus erythematosus (SLE) is complex and variability in practices exists. Guidelines have been developed to help improve the management of SLE patients, but there has been no formal evaluation of these guidelines. This study aims to compare the scope, quality, and consistency of clinical practice guidelines on the diagnosis, monitoring, and treatment of patients with SLE. Methods. Electronic databases were searched up to April 2014. The Appraisal of Guidelines for Research and Evaluation (AGREE) II instrument and textual synthesis was used to appraise and compare recommendations. Results. Nine clinical practice guidelines and 5 consensus statements were identified, which covered 7 topics: diagnosis, monitoring, treatment, neuropsychiatric SLE, lupus nephritis, antiphospholipid syndrome, and other manifestations of lupus. The methodological quality of the guidelines was variable, with the overall mean AGREE II scores ranging from 31% to 75%, out of a maximum 100%. Scores were consistently low for applicability, with only 1 guideline scoring above 50%. There was substantial variability in the treatments recommended for class II and V lupus nephritis, the recommended duration of maintenance therapy for class III/IV lupus nephritis (from 1 to 4 years), and timing of ophthalmologic examination for patients taking corticosteroids. Conclusion. Published guidelines on SLE cover a complex area of clinical care, but the methodological quality, scope, and recommendations varied substantially. Collaborative and multidisciplinary efforts to develop comprehensive, highquality evidence-based guidelines are needed to promote best treatment and health outcomes for patients with SLE.

Results. Eleven relevant questions were identified for the literature search. Based on the evidence from the literature, the expert panel recommended that each trial should report the following items: 1) disease activity response and disease activity states; 2) appropriate descriptive statistics of the baseline, the endpoints and change of the single variables included in the core set; 3) baseline disease activity levels (in general); 4) the percentage of patients achieving a low disease activity state and remission; 5) time to onset of the primary outcome; 6) sustainability of the primary outcome; 7) fatigue. Conclusion. These recommendations endorsed by EULAR and ACR will help harmonize the presentations of results from clinical trials. Adherence to these recommendations will provide the readership of clinical trials with more details of important outcomes, while the higher level of homogeneity may facilitate the comparison of outcomes across different trials and pooling of trial results, such as in meta-analyses.

В 2016 г., по истечении 10 лет после публикации, впервые были обновлены международные рекомендации Европейской антиревматической лиги (EULAR) по лечению подагры. Заключительный вариант обнов-ленных рекомендаций, включающий три базовых постулата и 11 позиций, оговаривающих принципы про-ведения симптоматической терапии острого приступа артрита и, что более важно, стратегии урат-снижаю-щей терапии, направлен на возможность их практического использования; однако некоторые, не лишен-ные неоднозначных суждений, положения требуют обсуждения и пояснения, в том числе исходя из рос-сийских реалий. The final version of the updated guidelines includes three overarching postulates and 11 key recommendations evaluating principles of the symptomatic therapy of an acute attack of arthritis and, more importantly, strategies of urate-lowering therapy. The aim of the above-mentioned guidelines is the possibility of their practical use. However, some statements, which contain controversial judgments, require further discussion and clarification, also in the context of russian realities. Так, если в предыду-щей версии три пункта были посвящены «общим» вопро-сам и немедикаментозным методам лечения, то в новой все они уложились в один-единственный пункт [1,4]. С другой стороны, отдельно были вынесены три клю-чевых постулата (см. таблицу), глобально отличающие мо-дифицированные рекомендации 2016 г. от версии 2006 г.Важно, что постулат А начинается с упоминания о не-обходимости максимального информирования пациента о причинах возникновения и о принципах лечения подаг-ры, как симптоматического, так и основного, к которому следует отнести урат-снижающую терапию. Интересно, что этот постулат имел максимальный уровень согласия экспертов, в сравнении со всеми другими пунктами реко-мендаций. Действительно, именно незнание пациентов с подагрой о возможности полностью справиться с болез-нью является основной причиной низкой приверженности урат-снижающей терапии и, как следствие, формирования тяжелой хронической тофусной подагры [5,6].
