EUS-guided biliary drainage: A systematic review and meta-analysis

Abstract: ERCP is the current procedure of choice for patients with jaundice caused by biliary obstruction. EUS-guided biliary drainage (EUS-BD) has emerged as an alternative to ERCP in patients requiring biliary drainage. The aim of the study was to conduct a systematic review and meta-analysis to report the overall efficacy and safety of EUS-BD. We conducted a comprehensive search of several databases including PubMed, EMBASE, Web of Science, Google Scholar, and LILACS databases (earliest inception to June 2018) to id… Show more

“…[ 34 ] In a more recent systematic review and meta-analysis including 1437 patients from 23 studies, a pooled rate of technical and clinical success of 91% (95% confidence interval [CI]: 87.7–94.2, I 2 = 76.5) and 87% (95% CI: 82.3–90.6, I 2 = 72.4), respectively, are shown. [ 35 ] The calculated pooled rate of re-intervention and adverse events were 6.5 (95% CI: 3.8–10.8, I 2 = 69.3) and 17.9% (95% CI: 14.3–22.2, I 2 = 69.1), respectively.…”

“…Adverse events occurred in 17.9%. The main adverse events were bile leakage (4.1%), stent migration (3.9%), and infections (3.8%) [ 65 ]. Although there were high success rates using interventional EUS, adverse events were higher than BE-assisted ERCP.…”

“…A recent meta-analysis of twenty-three studies including 1437 patients found a technical success rate of 92% and a clinical success rate of 87% of EUS-BD using mixed approaches in a variety of indications [ 13 ]. Pooled AE rate was reported with 17.9%.…”

“…Since then, EUS-BD has become an alternative to ERCP in cases that require BD [ 11 , 12 ] and in particular as a backup method if standard ERCP-approach fails. Various studies have reported high technical success rates associated with EUS-BD [ 11 , 12 , 13 , 14 , 15 ].…”
