Evaluating the Reliability of Neurological Pupillary Index as a Prognostic Measurement of Neurological Function in Critical Care Patients

Abstract: BackgroundNeurological pupil index (NPi) is a novel method of assessing pupillary size and reactivity using pupillometry to reduce human subjectivity. This paper aims to evaluate the use of NPi as a potential prognostic tool in a broad population of neurocritical care patients by observing the correlation between NPi, modified Rankin Scale (mRS), and Glasgow Coma Scale (GCS). MethodsOur data was collected from 194 patients in the neurosurgical intensive care unit (ICU) at Arrowhead Regional Medical Center (ARM… Show more

“…The Neurological Pupil index (NPi) is a dimensionless scale set between 0 and 5 that is calculated through an unpublished algorithm using all variables measured through the pupillometer. Through NPi, objective neurological prognostication is possible in critical care 19,20 . However, NPi can be altered under general anesthesia using propofol in adults, even in the absence of neurological deficits 15 .…”

“…In comparison to manual pupil assessment, pupillometry offers a more accurate and objective assessment, with only one-third of non-reactive pupils [24,[58][59][60][61] and half of anisocoria cases [8] being detected by practitioners. Additionally, concordance between the pupillometer and subjective assessment of pupil parameters was poor, with an 18% discordance in PLR and 39% discordance in measurements with small pupils (diameter < 2 mm) [8].…”
