Evaluation of an intra‐institutional diabetes disease management program for the glycemic control of elderly long‐term care diabetic patients

Abstract: No case of hypoglycemia was detected on the examination day, or during the preceding 3 weeks. No significant difference was found among the different Functional Outcome Swallowing Scale categories. These results are within satisfactory range for this category of patients suggesting that our diabetes disease management program contributes to a better glycemic control.

“…This prevalence rate was much lower than the prevalence rate of DM in the general U.S. population and was also lower than 7.78% prevalence rate noted in HEN patients from the United Kingdom . Other studies evaluating the prevalence of DM and hyperglycemia in patients receiving EN have focused on hospitalized patients or those residing in skilled nursing facilities and revealed much higher rates of DM (≈36%) consistent with their higher clinical acuity compared with our outpatient population …”

“…Other studies investigating DM medication use in skilled nursing home residents admitted to the hospital noted that 35% of patients were treated with insulin, 29% with OHGs, and 36% were diet controlled . Unfortunately, this study did not differentiate between patients receiving jejunal vs gastric feeds.…”

“…Apart from routine measuring at the baseline (T1), after the intervention (T2) and 3 months after the intervention (T3), frequent blood pressure and blood glucose monitoring will be encouraged to continue monitoring the health status (Lubart et al . , Thabit & Hovorka ). Thus, it is suggested that clients seek regular monitoring from their community centre throughout the period of study.…”
