Evaluation of endometrial thickness with transvaginal ultrasonography and histopathology in premenopausal women with abnormal vaginal bleeding

Abstract: An endometrial thickness >8 mm is more likely than that of 8 mm or less to be indicated with endometrial biopsy in premenopausal uterine bleeding. Pipelle endometrial biopsy is an accurate diagnostic procedure for the detection of high-grade endometrial lesions in premenopausal women.

“…63 Furthermore, it is 95% accurate in detecting endometrial abnormalities in symptomatic premenopausal women taking tamoxifen. 64 If endometrial sampling shows benign pathologic findings, recurrent AUB can be observed and followed up with periodic endometrial sampling to exclude endometrial disease.…”

“…5 AUB in perimenopausal women demonstrated prevalence of endometrial hyperplasia (11.8%), polyp (4.2%) and adenocarcinoma as (5.5%) respectively. 6 Endometrial hyperplasia is a known precursor for endometrial carcinoma and it can progress to more severe pathology which is time related. Less than 2% of hyperplasia without atypia progress to endometrial carcinoma in 23% over a mean duration of 4 years.…”

“…Normal endometrial echo complex thickness varies based on the patient's menstrual cycle typically ranging from 3-5mm at the end of menses to approximately 12-14mm in the secretory phase [21,22]. Based on correlative data of endometrial echo complex thickness and histopathology in premenopausal women with abnormal uterine bleeding published by Ozdemir et al [23] the average endometrial echo complex in premenopausal women with atrophy was 4.6 ± 2.5mm [23]. In our cohort, the respective mean endometrial echo complex thickness for all patients with atrophy and no intracavitary pathology was 6.4 ± 3.6mm.…”
