Evaluation of psychological stress, cortisol awakening response, and heart rate variability in patients with chronic prostatitis/chronic pelvic pain syndrome complicated by lower urinary tract symptoms and erectile dysfunction

Abstract: BackgroundMental stress and imbalance of its two neural stress systems, the autonomic nervous system (ANS) and the hypothalamic–pituitary–adrenal (HPA) axis, are associated with chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) and erectile dysfunction (ED). “…Our study reveals a significant relationship between elevated stress levels and increased intensity of pelvic pain among women with deep endometriosis [23][24][25][26]. This correlation suggests that stress exacerbates the painful symptoms of endometriosis, which may also manifest in related symptoms such as dyspareunia, dyschezia, strangury, and low back pain.…”

“…To the best of our knowledge, no previous study had investigated the relationships of both urinary and respiratory domains with psychological distress. Indeed, Bai et al studied the presence of psychological distress with the Beck Anxiety Inventory and Perceived Stress Scale in chronic prostatitis/chronic pelvic pain syndrome patients with LUTS [43]. Specifically, they identified a strong correlation between LUTS and severity of psychological stress [43].…”

“…Indeed, Bai et al studied the presence of psychological distress with the Beck Anxiety Inventory and Perceived Stress Scale in chronic prostatitis/chronic pelvic pain syndrome patients with LUTS [43]. Specifically, they identified a strong correlation between LUTS and severity of psychological stress [43]. Moreover, Guglielmi et al analyzed in truck drivers the relationship between sleep problems and mental health, scored with a general health questionnaire [9].…”

A biopsychosocial model also correlated pelvic floor dysfunction with LUTS, 66 and autonomic nervous system sympathovagal imbalance was found to be associated with chronic pelvic pain syndrome and psychological stress severity 67 …”
