Evaluation of renal biopsies in cats and dogs — histopathology in comparison with clinical data

Abstract: Histopathological findings of renal biopsies in cats and dogs with diffuse nephropathies generally lead to an exact diagnosis and facilitate prognostic judgements. Complications following renal biopsy are usually slight, provided the biopsy is performed properly. Routine renal laboratory data have been compared with histopathological findings. High urine protein values are often the result of glomerular lesions, whereas high creatinine values are frequently related to tubulointerstitial lesions. In general, th… Show more

“…Notably, the cats with specific renal diseases in this study were younger at death than the cats with nonspecific renal lesions. Previous studies have also found cats with nonspecific renal lesions to be older, 15,19 suggesting these lesions might represent a disease process that progresses more slowly. Chronic inflammation is often implicated in the development of fibrosis via mechanisms such as macrophage activation, so the correlation between interstitial inflammation and fibrosis scores is noteworthy.…”

“…Of the cats with azotemic CKD, those with specific renal diseases died at 11 (6, 15) years of age; those with nonspecific renal lesions died at 16 (13, 18) years of age. The nonazotemic cats died at 15 (14,19) years of age. Nine nonazotemic cats were hypertensive (reflecting active recruitment of hypertensive cats to the clinics), 9 were normotensive, and 2 had unknown blood pressure.…”

“…3,4 In human medicine, even in patients with primary glomerular disease, it is tubulointerstitial lesions that correlate most strongly with renal function. Recently feline studies have compared histopathological findings with stage of kidney disease and identified that the severity of tubular degeneration, interstitial inflammation, fibrosis and glomerulosclerosis was more marked in the later IRIS stages of CKD.…”

“…1 The most common histopathological finding was non-specific tubulointerstitial inflammation, fibrosis and mineralization referred to as tubulointerstitial nephritis. [1][2][3][4] The development and progression of these lesions will be the focus of this article but given the limited response of the kidney to an inciting injury, tubulointerstitial inflammation and fibrosis is a common end pathology for many primary renal diseases.…”

“…Reports published in both human and veterinary literature in the 1970s and 1980s demonstrate the impracticality of distinguishing between glomerular pathologies based exclusively on applied clinical and laboratory parameters [26]. Furthermore, it has been demonstrated that diagnosis and therapy are modified in about 40% of cases following renal biopsy [36] and that the technique used alters the clinical prognosis in about 50% of cases [40].…”
