Evaluation of Surgical Factors Affecting Vitreous Hemorrhage Following Port Delivery System With Ranibizumab Implant Insertion in a Minipig Model

Abstract: Supplemental Digital Content is Available in the Text. This minipig model of vitreous hemorrhage demonstrated that scleral dissection followed by thorough laser ablation of the pars plana before the pars plana incision could mitigate vitreous hemorrhage during Port Delivery System with ranibizumab implant insertion surgery.

“…As such, the PDS has gone through some optimization to help mitigate some of these risks, such as improving the implant insertion procedure with laser ablation of the pars plana at the scleral incision site, diminishing the rate of vitreous hemorrhage. 19,28 The ongoing Phase III trials should elucidate the safety profile of the PDS further.…”

“…Fibrosis adjacent to the implant was observed in both treated animals and controls ( Figure 2), and thus appeared to be due to instability of the implant, which is not optimized for the structure of the pig eye. 15,21 Furthermore, the implant also required suturing to hold it in place, exacerbating the local response. Overall, the PDS implant and associated procedures, such as refilling, were well tolerated in this species.…”

“…Since vitreous hemorrhage resolved without intervention within a month of implantation in the majority of animals, and did not limit assessment of safety endpoints during the toxicology study, we developed a minipig surgical model of vitreous hemorrhage where various surgical techniques were evaluated. 15 Another challenge for a drug/device combination product for retinal diseases is related to biocompatibility testing standards, such as ISO 10993. Because ISO 10993 does not apply to ocular implants, it was utilized as a starting point, along with ISO 11979 and ANSI Z80.7 standards.…”

“…A minipig surgical model for vitreous hemorrhage with ranibizumab PDS implant was useful for investigation of the insertion of long-acting, indwelling devices into the vitreous cavity and provided important information that may be relevant to similar surgical approaches in different settings. 34 As discussed further in the section on pathology reports, intravitreal injection of some saline solutions in minipigs can cause retinal and other ocular changes that can be difficult to discern from test article-related findings. 35 Minipigs have not been used for intracameral toxicity studies to the author's knowledge, and they have a smaller anterior chamber angle opening distance and angle recess area, which are disadvantages to the more similar angle size in dogs and humans.…”
