Evaluation of the efficacy and safety of adjuvant treatment to levodopa therapy in Parkinson´s disease patients with motor complications

Abstract: Compared to placebo, adjuvant therapy reduces off-time, levodopa dose, and improves UPDRS scores in PD patients who develop motor complications on levodopa therapy. However, this is at the expense of increased dyskinesia and numerous other side-effects. Indirect comparisons suggest that dopamine agonist therapy may be more effective than COMTI and MAOBI therapy, which have comparable efficacy. However, as indirect comparisons should be interpreted with caution, direct head-to-head randomised trials assessing t… Show more

“…Initially, 30 publications (published from 1996 to 2012) concerning entacapone, describing a total of 13 distinct randomised, double-blinded and placebo-controlled clinical trials, were considered. For the period up to 2008 these results matched with the findings of Stowe et al 12 Eventually, four entacapone studies were included for the meta-analysis (see Table 1). The four studies are 17 This last trial 17 recruited both fluctuating and non-fluctuating patients, but separate study results on the fluctuating patients were available; therefore, only those parts of the study results were included, whenever available (see Table 1).…”

“…Однако в последние годы чаще используют быструю схему замены препаратов, которая предполага-ет одномоментное переключение с одного препарата на эквивалентную дозу другого препарата. Привлека тель-ность данной схемы определяется не только ее просто-той, но и отсутствием опасности ослабления эффектив-ности терапии и синдрома отмены АДР [26][27][28].…”

“…However, in a proportion of patients, particularly those with less-advanced disease, the addition of a COMT inhibitor to an optimised levodopa dose can lead to the emergence of dyskinesia. 33,34 Therefore, in these cases the addition of safinamide, with its favourable profile in terms of improving wearing-off symptoms without increasing dyskinesia, could be an effective approach.…”
