Evaluation of the efficacy of rotary vs. hand files in root canal preparation of primary teeth in vitro using CBCT

Abstract: Rotary files prepared more conical canals in primary teeth than manual instruments. Reduced preparation time with rotary files enhances patient cooperation especially in young children.

“…According to the present results, the null hypotheses for root canal volume and surface need to be rejected, since root canal preparation with both rotary file systems resulted in significantly greater increase in root canal volume and surface in both primary teeth and permanent teeth. These results confirm two recent studies evaluating the shaping effectiveness of different rotary files vs hand files in primary teeth . Presumably, the greater conicity of the rotary instruments compared with conventional hand files contributed to more dentine removal in the coronal region, resulting in greater post‐preparation root canal volume and surface area.…”

“…5 Rotary files were first evaluated in primary teeth by Barr et al, 6 who concluded that root canal preparation was faster with rotary files than with manual files, and produced uniform and predictable canal morphologies. Although few other studies have evaluated the performance of rotary file systems in the primary dentition, [7][8][9][10][11][12][13] their outcomes are still limited to provide an exact conclusion.…”

“…Our results support the concept that endodontic treatment in primary dentition should follow the same biological principles applicable to permanent teeth, and manual or rotary instrumentation techniques are safe procedures for primary molars without significant root resorption [27]. Advantages of the LSTR concept [5] such as less chair time, single-visit treatment and providing an option for poor-prognosis molars, might be considered in public service dentistry, but its cost-effectiveness deserves further investigation.…”
