Evaluation of the Efficacy of Endoscopic Treatment for Gastric Cancer Considered in Terms of Long‐term Prognosis

Abstract: A comparative evaluation was made between the efficacy of endoscopic treatment in 116 cases with that of surgical treatment in 59 cases. Both groups were treated at Yamagata Prefectural Central Hospital during a period from July 1978 up until December 1989. All of the patients had single mucosal adenocarcinoma of a well differentiated type which were not accompanied by ulcer or ulcerative scar with their maximum diameter of foci being less than 20mm. A curve of the survival rate of both groups as of June 30, 1… Show more

“…These findings are consistent with those of previous studies showing similar survival rates in EGC patients undergoing EMR and surgical treatment. [8][9][10] Those previous studies, however, were limited by the small numbers of included patients and by baseline differences between the EMR and surgery groups. Our findings are subject to selection bias and confounding with respect to the relative severity of preprocedure risks among patients who underwent EMR and those who underwent surgery.…”

“…Endoscopic treatment for early gastric cancer has been widely employed in many institutions throughout Japan. 1–3 We started our prospective study on endoscopic treatment for early gastric cancer in September 1994 in 42 institutions in Japan (see Appendix I), and we plan to finish the study in 2002. The aim of this study is to clarify the present status and problems inherent to treatment using endoscopic mucosal resection (EMR) 1,2 and other endoscopic modalities and to investigate the possibility of extending the indications of endoscopic treatment.…”

“…As it becomes increasingly important for surgeons to consider the patients’ quality of life, the indication for minimally invasive surgery should be enlarged, especially in early gastric cancer 6 , 7 . However, it is difficult to diagnose the depth of cancerous invasion preoperatively with reasonable certainty.…”
