Evaluation of the EIE-IgM-Leptospirose assay for the serodiagnosis of leptospirosis

Abstract: Access to low-cost, effective diagnosis for leptospirosis is urgently needed in developing countries. The EIE-IgM-Leptospirose, a kit produced for public health laboratories in Brazil, was shown to have a sensitivity of 76% (77 of 102 patients) and 100% (102 of 102 patients) during acute and convalescent-phase leptospirosis, respectively, and a specificity of 93-100% (total healthy and patient control subjects evaluated, 486). These findings indicate that the assay will be useful for diagnosis of this emerging… Show more

“…Our evaluation was conducted from a single site and therefore may not necessarily apply to other urban settings of endemicity. However, the study's findings are consistent with those of evaluations of in-house assays or individual commercial kits which were performed with predominantly urban-based subjects from other sites (3,22,25,28,32).…”

“…38 The anti-Leptospira immunoglobulins develop during the first 10 d following infection, the IgM peak is quickly followed by increasing IgG levels. 3,7 The antibodies persist in patient serum for up to five years 41,42 and even low antibody levels are protective. 3,40 Conversely, a humoral immune response with a high titer of agglutinating antibodies does not protect cattle, rather a cellular immune response is required for protection against leptospirosis.…”

“…Blood samples were tested for the presence of anti- Leptospira IgM antibodies by an ELISA (Bio-Manguinhos, Rio de Janeiro, Brazil) (McBride et al, 2007). This assay is used by the Brazilian Ministry of Health for laboratory confirmation of leptospirosis and has a sensitivity of 100% in identifying antibodies during the convalescent phase of the illness among cases confirmed by the standard MAT.…”
