Evaluation of the levels of trace elements in the blood and hair of female patients with chronic telogen effluvium

2-3 months after a stressor, such as stress, medications, trauma, endocrine disorders, nutritional deficiencies, event feverish conditions, telogen effluvium (TE) is characterised by diffuse hair loss. Zinc is an essential cofactor for more than 300 hair follicle enzymes with crucial physiological roles. The purpose of this research was to determine the zinc concentration in the serum of TE patients. This was a case-control study with 25 patients with TE (Group A) and 20 persons of the same age and gender who seemed to be healthy (Group B). Zinc levels in the Serum of all participants were evaluated. Comparing the sick group to the control group, serum zinc levels were insignificant.

AimThe etiology of telogen effluvium (TE) includes situations that may cause physiological stress, surgical trauma, inflammatory, infectious, iatrogenic causes, medications and nutritional deficiencies. TE has been associated with iron deficiency, vitamin B12 deficiency and thyroid diseases. In recent years, the use of over‐the‐counter food supplements containing vitamins and minerals such as biotin, vitamin D, zinc (Zn), copper (Cu) and selenium (Se) has been increasing in TE patients. The aim of this study is to investigate whether there are differences in nutritional status, vitamin and mineral levels by comparing individuals with TE and a control group.Materials and MethodsThis case–control study included 90 female patients diagnosed with chronic telogen effluvium (CTE), and 90 female controls volunteered to participate in the study who consulted for reasons other than TE. Both groups aged 18 and over and applied to dermatology polyclinic between 01.09.2022 and 01.09.2023. A detailed anamnesis was taken from all patients, a hair pull test was performed, and TE was diagnosed after a dermoscopic examination was performed on all areas of the scalp. Then, serum vitamin D, Zn, Cu, Se levels and biotin levels in serum and urine were measured. Hemoglobin (Hb), ferritin, vitamin B12 and thyroid function tests were retrospectively scanned from the hospital database.ResultsIt was determined that Zn levels were significantly lower in CTE patients than in controls. Se levels were found to be significantly higher in patients than in controls. There was no difference in Hb, ferritin, vitamin B12, thyroid function tests, vitamin D, Cu levels, serum and urine biotin levels between the two groups. Zn, Cu/Zn and Se levels were found to have statistically significant diagnostic performance in predicting the diagnosis of CTE. Cu/Zn ratio and Se value were found to be significant predictors of CTE.ConclusionThis study shows us that nutritional deficiencies are not as common as thought in patients diagnosed with TE. Other causes that may cause TE should be investigated by a detailed anamnesis and a good physical examination. After all, tests for suspected conditions should be performed and individualized treatment options should be created for each patient.

Chronic Telogen Effluvium (CTE) is a common non- scarring, diffuse type of hair loss, affecting a significant proportion of population. The purpose of the current study is to assess the relationship between CTE and trace metal levels, specifically copper and iron. Objective: To collect hair and serum samples from both control group and patients with CTE and to analyze the association between hair copper and iron content and serum copper and iron levels in both groups. Methods: The Dow University Hospital (DUHS) in Karachi, Sindh, Pakistan, served as the site of this case control research. The duration of study was one year. The study population encompassed hundred individuals, categorized into 50 cases with CTE and 50 healthy controls. Using non probability sampling technique (convenient sampling), the sample size was determined. Diagnostic criteria a positive hair pull test. Hair and serum samples were collected from both groups for trace metal analysis. Analysis was carried out using ab Atomic Absorption Spectrophotometer (Perkin Elmer Analyst 800, located in Waltham, MA, USA). Data analysis was carried out using SPSS® software for Windows® version 16.0. Results: Results showed that hair samples from patients had considerably lower iron levels than those from controls (p<0.001), while there was no significant difference in copper levels (p=0.713). Additionally, the CTE group showed noticeably greater copper levels (p<0.001) and significantly lower serum iron levels (p<0.001). Conclusions: The study identified a positive association between iron deficiency and hair loss.
