Evaluation of Third‐Generation Screening and Confirmatory Assays for HCV Antibodies

Abstract: A third-generation (gen.) screening and immunoblot assay (Ortho EIA-3.0; Chiron RIBA-3 prototype), using antigens derived from the capsid and different nonstructural regions (NS3, NS4 and NS5) of the hepatitis C virus viral genome, were evaluated in comparison with the corresponding second-gen. assays (Ortho EIA-2.0; revised Ortho EIA-2.5; Chiron RIBA-2). In 203 depository sera of blood donors, positive in EIA-2.0, specificity of the screening assays was improved as shown by an increase in positive predictive … Show more

“…Although false-reactivity with EIA systems has been reported in other studies, 11,14,15 the high discordance rate between the EIA-3 and RIBA-3 results in our study led us to assess whether there was a pattern to the false-reactive HCV EIA-3 test results. We found no association between demographic characteristics and false positivity with the HCV EIA-3.…”

“…This explains the reason for including recombinant NS3 in the first [45][46], second [47] and third generation ELISA [48] for detecting anti-HCV-antibodies.…”

“…The sensitivity and specificity of these 3 rd generation kits depend on the type of antigen used. 21 With the advent of the molecular techniques, the circulating virus (viraemia) can now be detected by HCV RNA measurement. This testing is used for early detection (before seroconversion) and also is essential for confirmation of active HCV infection and monitoring of antiviral therapy.…”

“…This second generation ELISA assay had a higher sensitivity and specificity over the earlier one. 21 At present, the third generation ELISA assays use highly purified antigens with the addition of NS5 region of HCV genome and have the highest sensitivity and specificity. 20 The antigens are either a purely recombinant (Ortho), or purely synthetic peptides (UBI), or a mixture of recombinant and synthetic peptides (General Biologicals).…”
