Evaluation of tracheal intubation in several positions by the Pentax-AWS Airway Scope: a manikin study

Abstract: Although tracheal intubations with AWS in all five positions tested were successful, intubation with the patient in the Sitting, Right-LT, and Prone positions was more difficult and required more time than that in the Supine position.

“…In addition, the semirecumbent position effectively prevented hypoxemia due to increased compression of the lungs as well as supine hypotensive syndrome. The Airway Scope is useful in a variety of positions (Komasawa et al 2010) and, as in previous reports, (Suzuki et al 2009) we were able to perform successful tracheal intubation using the Airway Scope in a semirecumbent position. We elected to maintain spontaneous respirations because controlled ventilation would have been difficult owing to elevated intra-abdominal pressure during tumor fluid extraction.…”

“…We included 22 manikin studies (42 trials) . The Pentax AWS was more effective than the Macintosh laryngoscope for all outcomes except the rate of audible dental clicks or significant dental compression (equivalent to oral or pharyngeal injury in human studies, Table ).…”

“…Interestingly, a recent study by Komasawa et al found slower times for successful sitting face-to-face intubation using the Pentax AWS Scope (Pentax Ricoh Imaging Co., Ltd., Tokyo, Japan) than in various other positions (29). However, residents who participated in the study had previously intubated only in the standard position and were not taught the face-to-face technique beforehand.…”
