Evaluation of Ultrasound Elastography Combined With Chi-Square Automatic Interactive Detector in Reducing Unnecessary Fine-Needle Aspiration on TIRADS 4 Thyroid Nodules

Abstract: BackgroundConventional ultrasound diagnosis of thyroid nodules (TNs) had a high false-positive rate, resulting in many unnecessary fine-needle aspirations (FNAs).ObjectiveThis study aimed to establish a simple algorithm to reduce unnecessary FNA on TIRADS 4 TNs using different quantitative parameters of ultrasonic elasticity and chi-square automatic interactive detector (CHAID) method.MethodsFrom January 2020 to May 2021, 432 TNs were included in the study, which were confirmed by FNA or surgical pathology. “…The complex tissue composition of malignant nodules often leads to calcification and necrosis, resulting in heterogeneous shear wave elastograms, potentially biasing elasticity measurement outcomes. 25 The “stiff rim sign”, derived from the principle of fibroblast proliferation by cancer cells, renders the tumour periphery harder due to its exclusion of the nodule’s interior condition and resistance towards internal calcification/necrosis influences on elasticity quantification, unrestricted by cutoff values. 26 , 27 This study found the “Stiff rim sign” to exhibit superior diagnostic prowess amongst three thyroid cancer detection methods using shear wave elasticity.…”
