Evaluation of uterine artery embolization on myoma shrinkage: Results from a large cohort analysis

Abstract: Objective:There are still contradictory opinions on the success rates of uterine artery embolization (UAE) for the treatment of myomas. In this scenario, our study aims to assess the effect of UAE on myoma shrinkage.Materials and Methods:The study included 337 women in reproductive age affected by a single symptomatic intramural myoma and declined surgery, undergoing UAE. The uterus and myoma diameters and volumes were determined on ultrasonographic scans before and 3, 6, and 12 months after the procedure.Resu… Show more

“…As the uterus receives accessory blood flow, normal uterine tissue heals from the ischemic shock. In contrast, fibroids are unambiguously damaged, which results in ischemic damage with necrosis and finally an irreparable reduction in fibroid size [ 43 , 44 ].…”

“…The occurrence of myometrial/perivascular fibrosis may impair effective reduction of fibroid size and increase the risk of arteriosclerosis. We need careful follow-up of these women with fibroids even after uterus-sparing surgery such as uterine artery embolization [ 67 ]. Based on our current findings, we postulate that use of anti-fibrotic agents either alone or in combination with GnRHa may be the potential target in the treatment of fibroids.…”

“…UAE is a minimally invasive treatment, where the blood supply to the fibroids is blocked by using embolizing material under fluoroscopic guidance performed by an interventional radiologist [8,24,25]. The fibroid, lacking the blood supply, eventually shrinks without a negative impact on fertility [24,25].…”
