Evaluation Study of Congestive Heart Failure and Pulmonary Artery Catheterization Effectiveness

Abstract: DVANCES IN MEDICAL THERAPYhave improved outcomes for many ambulatory patients with heart failure and low ejection fraction (EF). [1][2][3][4] However, each year an estimated 250 000 to 300 000 patients are hospitalized for heart failure with low EF, 5 and the 1-year survival rate after hospitalization may be as low as 50%, even with recommended medical therapies. 6,7 In nonrandomized studies, patients undergoing therapy with vasodilators and diuretics to reduce filling pressures to near normal levels have had … Show more

“…As a result, patients are now less likely to require hospitalization for the once‐typical course of decompensation reversed with simple intravenous diuresis 15, 16, 17, 18. The contemporary population of hospitalized HF patients is now older, with a longer duration of disease and a larger burden of right HF, cardiorenal syndrome, and noncardiac comorbidities.…”

“…20 In contrast with the ESCAPE trial, this pilot study evaluated a higher-risk population with LCOS and renal insufficiency. However, we did not find a difference between the groups with and without a pulmonary artery catheter.…”

“…In the ESCAPE trial, AHF treatment guided by pulmonary artery catheter was non‐useful compared with conventional therapy 26. It is noteworthy, however, that physicians treating ESCAPE trial patients had central venous pressure measurements and echocardiograms available on a daily basis in both groups, although their role for guiding treatment was not clearly reported.…”
