Evidence‐Based Choice of Esophageal Stent for the Palliative Management of Malignant Dysphagia

Abstract: Self-expanding metallic stents are superior to plastic stents in terms of stent insertion-related mortality, morbidity, and quality of palliation. The uncovered variety is disadvantaged by high rate of tumor in-growth; adequately designed randomized, controlled trials need to examine outcomes and cost-effectiveness of covered versus uncovered metallic stents.

“…According to a meta-analysis by Yakoub, compared to plastic stents, metal stents are associated with significantly reduced stent-related mortality (1.7% versus 11.1%), reduced esophageal perforation (1.4% versus 9.4%), and stent migration. On the other hand, tumor ingrowth through the open mesh architecture may occur in 13% of SEMS cases compared to 1.6% for when plastic stents are used [12, 13]. In response to this, the next generation of partially covered self-expanding metal stents, which added a thin silicone or plastic covering to the body of the stent to prevent tumor ingrowth was developed.…”

“…The migration risk increase when the stent is placed across the gastroesophageal junction [22]. Partially covered SEMS help to prevent migration but their removal is sometimes difficult according to high rate of tumor in growth [18,31].…”

“…Different types of stents are available including plastic stent and metal stent. A major complication of plastic stent was immigration and weren't very useful and effective for patients with esophageal neoplasm [17][18]. Self-expanding metallic stents [SEMS] had fewer complications compared to plastic stents [10,17].…”
